Main duties of the job
* To undertake holistic OT assessment, clinical diagnosis and individualised treatment plans for patients on the caseload.
* To contribute to the inter-disciplinary working within the service, providing an effective, high-quality day to day service provision to the active client group.
* Rotations to include:
o Farnham inpatient rehabilitation.
o Enhanced Recovery and Support at Home.
* There is potential for further rotations to be added within future scope.
* Hours include rostering for 7-day working with hours of work varying from 8-4 to 10-6 during the week, which can vary across rotations.
Job responsibilities
* Take responsibility for managing a varied patient caseload and follow the OT process, seeking advice on the more complex cases to ensure the provision of a timely, clinically effective service based on evidence-based practice.
* To provide holistic OT assessment, goal setting, individualised treatment programmes, care planning and delivery and review within intermediate care; adapting as the patients' needs evolve.
* To provide standardised cognitive assessments and treatment programmes as required.
* To provide environmental and functional assessments, composing & implementing appropriate treatment programmes.
* To educate and provide necessary interventions around management of anxiety and fatigue.
* To identify, assess and manage risk.
* Demonstrate a sound understanding of clinical risk and report untoward incidents both verbally and in writing using the Trust reporting system
* To work closely and jointly at times with Physiotherapy, Nursing colleagues and the rest of the MDT to ensure the patient receives holistic treatment.
Person Specification
Qualifications
* HCPC Registered OT
* Degree or Equivalent in Occupational Therapy
Experience
* Experience of implementing a person-centred intervention programme.
* Experience of referring on to other agencies including the voluntary sector to continue patient's care (could be a as a student).
* Documented evidence of CPD.
* Experience of work as an OT or placement within a community setting.
Skills & Knowledge
* Effective written and verbal communication skills with internal & external colleagues, patients & family members.
* Ability to manage own workload, determining priorities whilst balancing conflicting demands.
* Be able to appropriately contribute & participate in MDT settings.
* Ability to assess, plan, and evaluate the rehabilitation needs of patients within scope of practice.
* Knowledge & understanding of health legislation in current practice and clinical governance.
* Knowledge of risk assessment through varying elements of clinical practice.
* Involvement in project work within the team

* Hold a full driving license, have access to a car & have business insurance on this.
