Job summary
We are seeking an innovative, enthusiastic, and motivated Band 6 Associate Community Matron on a 30-hour permanent contract to join our Urgent Community Response Team, working flexibly across the Central and East Exeter locality. We are an integrated health and social care team that facilitates crisis management in the community, aims to prevent hospital admissions, and supports appropriate discharges, helping people remain as independent as possible at home. This fast-paced and dynamic team offers daily challenges and learning opportunities. This position is ideal for an experienced nurse committed to providing compassionate, holistic patient care while working flexibly across the area.

The Associate Community Matron is a vital member of the Integrated Health & Social Care team, supporting Primary care, Community Nursing, and the Multidisciplinary Team. The role involves using clinical assessment skills and expert knowledge to perform comprehensive and focused assessments within your scope and professional standards for patients with various acute and long-term conditions, including frailty. Flexibility, excellent time management, and strong communication and interpersonal skills are essential. You must be able to work autonomously with minimal supervision, including lone working, while being a committed team member.
The role also involves supporting the professional development of junior staff, focusing on their competencies and skills, and applying clinical leadership to care for patients with complex needs. You will collaborate with Managers, Senior Nurses, and Clinical Leads to develop the service according to patient needs and Trust-wide initiatives.
