Job Summary
This post supports the Lead Pharmacist for Care of the Elderly in the provision of a clinical pharmacy service to the care of the elderly wards at the Royal Cornwall Hospital, West Cornwall Hospital, as well as specialist provision of end‑of‑life care in hospices. The care of the elderly pharmacist works alongside other members of the multidisciplinary team, including consultants, junior doctors, specialist nurses, specialist pharmacists, junior pharmacists and pharmacy technicians to provide a robust clinical service.
This role can be started as Band 6 and moved to Band 7 once objectives have been met.
Main Duties of the Job
* Providing a core clinical pharmacy service to Care of the Elderly inpatients in line with the Trust's clinical pharmacy standards.
* Ensuring high quality, patient‑centred, and cost‑effective pharmaceutical care from admission through inpatient stay and successful discharge, including transfer of key information to primary care providers where appropriate.
* Providing a clinical pharmacy service to hospices as defined by our service level agreements.
* Actively providing medicines information and medication counselling to health‑care professionals, patients and/or carers.
* Working with multiprofessional teams across the care of the elderly speciality to produce and review medication guidelines that promote rational, economical and evidence‑based prescribing; with an appropriate monitoring practice to ensure policies and guidelines are adhered to.

Person Specification
Essential Qualifications
* Masters degree in Pharmacy
* Registered pharmacist with the General Pharmaceutical Council
* Post‑Graduate Diploma in clinical pharmacy (or equivalent) or working towards it
Desirable Qualifications
* Registered Independent Prescriber (or willing to train)
* Member of the Royal Pharmaceutical Society
Essential Experience
* Significant post‑registration experience covering a range of specialties
* Experience as a clinical pharmacist in an acute hospital setting
* Clinical supervision of junior staff
Desirable Experience
* Experience of deputising for a more senior clinical pharmacist
* At least RPS Advanced Stage I (Foundation) or equivalent in care of the elderly
* Clinical research experience or willingness to undertake
* Experience of training staff using coaching or teaching methods
