Location: This is a nationwide role, covering our existing and new portfolio of homes. The ability to drive and access to a car is essential
Contract type: Full time, 40 hours per week (Monday to Friday)
Rate: £40,000 per annum, plus £3,000 car allowance

About the role
No two days will be the same, but your day-to-day responsibilities will include:
* Support our care homes to complete clinical pre-admission assessments, travelling to different care settings nationwide within the allocated working day.
* Produce individual cost per care assessments that demonstrate the specialist care package, justification for interventions and costs, completed during working hours (office or remote from home).
* Coach Clinical Nurse Managers and Home Managers in the assessment and costing process and be involved in delivering training.
* Support the processing of referrals, working with the Referrals and Occupancy Management Team as requested by the Senior Clinical Assessment Specialist.
* Uphold the company vision and drive Exemplar Health Care’s strategy.
* Where required, support the reassessment of existing service users.
* Build relationships and opportunities with new and potential referrers.
* Attend conferences, events and networks to raise Exemplar Health Care’s profile as required.

About you

* An ability to conduct a thorough assessment of individual need and identify suitable packages of care to enable people to achieve their desired outcomes.
* Excellent verbal and written communication skills with all levels of staff and stakeholders.
* A passion for working with individuals with complex health needs.
* A desire to work as part of a fast-paced, responsive team, to develop a systematic approach to referral management and ensure the highest standard of customer experience.
* Flexibility – the ability to work independently and as part of a team; good organisation skills; self-motivation.
* Willingness to travel to various portfolio homes.
* Confidence in representing Exemplar Health Care at internal, commissioner and contract meetings.
Qualifications: To apply for this role, you must have a first-level nursing registration with a valid Nursing and Midwifery Council (NMC) PIN. We’re looking for Registered Nurses from all backgrounds (Adult, Mental Health and Learning Disability Nurses).
