The Care Co-ordinator is responsible for care planning, staff allocation, rota management and effective communication within the organisation.
Care Coordinator Key Responsibilities
• Care Assessments and Planning
• Support assessments for new and existing service users.
• Maintain, update and review personalised care plans.
• Staff Scheduling and Rota Management
• Respond to staff absences and ensure continuity.
• Service Delivery & Operational Support
• Ensure shifts and visits run smoothly.
• Support the Registered Manager in daily operations.
• Quality Assurance and Compliance
• Conduct spot checks, audits and client reviews.
• Report incidents and safeguarding concerns.
• Staff Support and Supervision
• Provide communication, guidance and updates to care staff.
• Assist with performance monitoring.
• On-Call Responsibilities
• Participate in on-call rota and emergency response.
Care Coordinator Person Specification
Essential:
• Previous care coordination or scheduling experience.
• Strong organisational and communication skills.
Desirable:
• NVQ Level 3.
• Experience in domiciliary care.
• Highly organised
• Manage own work with minimal supervision.
