Job Summary
This post supports delivery of the integrated respiratory service within Hillingdon borough. The appointee will join the existing multi-disciplinary respiratory team and work collaboratively with healthcare professionals and stakeholders to ensure joined‑up care for patients across acute, community, primary and secondary settings.
Suitable Candidates
Physiotherapist or Nurse with a sound respiratory knowledge base, interested in contributing to Integrated Respiratory Care for patients with chronic respiratory conditions. Candidates should have good working knowledge in diagnosis and management of asthma and COPD, confident clinical assessment skills, and experience working as an autonomous practitioner.
Main Duties
* Help develop and evaluate an integrated respiratory care service.
* Provide spirometry in the community setting.
* Provide multi‑disciplinary support to secondary care integrated COPD clinics.
* Develop and maintain guidelines and protocols relating to respiratory integrated care.
* Develop and implement strategies for effective management of chronic respiratory diseases as part of integrated care.
* Provide education and expert resource to practice nurses, the wider multidisciplinary team and general practitioners.
* Participate in Integrated Care MDTs and case conferences.
* Contribute to and support admission avoidance and early supported discharge schemes.

Person Specification
Criteria
* Diploma or degree in Physiotherapy/Nursing
* Current NMC/HCPC registration
* Teaching Certificate
* Full driving licence and access to a car
* Experience of caring for respiratory patients
* Knowledge of BTS/NICE/GOLD guidelines
* Sound respiratory assessment and clinical skills
* Evidence of CPD
* Non‑Medical prescribing
* ARTP certification
* MSc or evidence of equivalent experience/post‑graduate courses
* Ability to perform arterial/capillary blood gases
* Previous experience of integrated care, pulmonary rehabilitation and domiciliary oxygen
