Overview
Do you have a sound knowledge of respiratory conditions and how integrated working can be used to support this client group? Are you passionate about empowering your patients? We are Kent’s leading provider of NHS services in the community and we are looking for a highly motivated respiratory nurse to join our specialist respiratory team in East Kent.
Our Team Motto is to “work in partnership to make a positive difference for those living with respiratory conditions”. If you successfully secure this post you would work alongside an established multi-disciplinary team and hold a caseload of complex patients with a range of respiratory conditions that require specialist input. You would autonomously manage the assessment, planning, delivery and evaluation of patient care and utilise a whole systems approach when supporting admission avoidance for this client group.
Responsibilities
* Deliver excellent patient care along a care pathway across organisational boundaries and act as a specialist resource for the wider multi-disciplinary team across the health economy.
* Put in place management plans for patients who have complex respiratory conditions.

* Venturing into a 22.5 hour per week role with consideration for a band 6 or a band 5 development post if there is a demonstrated passion for respiratory care and a drive to make a difference in the application/interview process.
* Flexible working, commuting within East Kent (Thanet locality as base) and travel to other areas as required.
* Must be able to drive and have access to a car for the role.
* Experience as a respiratory nurse and the ability to work across organisational boundaries is preferred.
Working Pattern & Location
The post is 22.5 hours per week; additional working pattern is 22.5 hours between Monday and Friday (pattern to be agreed). We have bases in four localities across East Kent; the successful applicant will be based in Thanet but flexibility to travel is expected.
