The Role
The purpose of the service is to reduce inequalities within health, employment and life experiences for people with learning disabilities and mental ill health. To engage, enable and empower individuals with learning disabilities and mental ill health to have fulfilled, rewarding and healthy lives. Utilising a strengths-based approach, through partnership working with the individual, their family and carers, health partners, local businesses, employers, organisations and the local community we endeavour to achieve our purpose. Enabling the individual to live a life that matters to them.
Key Responsibilities
* To work as part of a team in order to support, monitor and develop the quality standards set by both local and national government
* To attend to the holistic needs of the individual in line with agreed care plans/programmes relevant to specialised services
* Carry out personal care and as a key worker, develop a relationship with key service users to enable them to feel supported so that they can achieve maximum independence
* Encourage the promotion of independence, equality, privacy, choice, respect and dignity of the service user/carer by reassurance and encouragement to enable the management of anxiety, regaining of confidence and maintenance/achievement of independence where possible
* Contribute to the evaluation of the risk to each service user within the establishment and also in the wider community
* To assist service users outside the establishment in accordance with the service users assessed needs, abilities and goals. This will include supporting with home visits in order to facilitate a safe transition to the person’s own home
About You
* Willing to work a rota system which is subject to variation and dependent on the needs of the service
* Willing to work additional hours as required. Relevant life or work experience in a care situation
* To take an active role in your own personal development and willingness to undertake QCF Level 2 or equivalent, manual handling, first aid and any other training required for the post
* To have a positive interest in working with people who have learning disabilities
* To act calmly and responsibly in an emergency situation and/or stressful situation
