Job Summary
We are recruiting a dedicated Band 6 Senior Community Nurse to join our community nursing team covering Woking. The team visits patients at home and in care homes, with opportunities for training and leadership development. The service operates 7 days a week, 8:00 am to 8:00 pm, with flexible shifts, including weekends and bank holidays.

Salary and Contract
Salary ranges from £39,205 to £47,084 annually, including a 5% High Cost Area allowance. This is a permanent, full-time or part-time role with flexible working options.

Job Responsibilities
* Manage a caseload of patients, providing full nursing assessments and care planning
* Act as the named District Nurse for designated GP practices
* Monitor medication regimes and ensure safety and compliance
* Maintain accurate patient records, both paper-based and electronic
* Lead and supervise team members, including mentoring and appraisals
* Participate in clinical audits and service development initiatives
* Build strong relationships with multidisciplinary teams and community stakeholders
Person Specification
Qualifications
* Current 1st level registration (Registered Nurse)
* Degree in relevant health care (or working towards)
* CPD evidence, specialist courses, and supervisory qualifications
Experience & Skills
* Minimum 2 years post-registration experience in community nursing
* Excellent assessment, mentoring, and leadership skills
* Effective communication and organisational skills
* Ability to work under pressure and across a 7-day week
Additional Requirements
* Valid UK driving license and access to a car
* Commitment to professional development and service improvement
