Job responsibilities
* Conduct structured medication reviews, particularly for patients with polypharmacy or complex long‑term conditions.
* Provide patient‑facing and telephone consultations to address medication queries, adherence, and side effects.
* Support the management of chronic conditions such as diabetes, hypertension, asthma, and COPD through medicines optimisation.
* Where qualified, prescribe independently within scope of practice and in line with practice policies.
* Process and manage repeat prescriptions and medication requests in line with practice protocols.
* Undertake medicines reconciliation following hospital discharge and outpatient correspondence.
* Identify and resolve prescribing issues, including drug interactions, contraindications and safety alerts.
* Support cost‑effective prescribing and adherence to local formularies and prescribing guidance.
* Monitor and implement national and local safety alerts relating to medicines.
* Participate in clinical audit and quality improvement initiatives.
* Support the practice in achieving quality targets and performance indicators related to prescribing and medicines management.
* Work collaboratively with GPs, nurses, and administrative staff to improve patient care pathways.
* Provide clinical advice to practice staff on medicines‑related issues.
* Contribute to practice meetings, clinical discussions, and service development planning.
* Provide education and support to patients to promote safe and effective use of medicines.
* Offer training and guidance to practice staff on prescribing processes and medicines management where appropriate.
Experience
* Registered pharmacist with the General Pharmaceutical Council
* Experience of working in primary care
Qualifications
* Masters degree in Pharmacy (MPharm) or equivalent
* Registered pharmacist with the General Pharmaceutical Council
* Independent Prescriber qualification or willingness to work towards it
* Experience working within a GP practice or primary care network
* Familiarity with GP clinical systems (e.g., EMIS or Vision)
