Aberdeenshire Discharge Coordinator - ARI
Band 6 - 30 Hours per week - Permanent
Aberdeenshire Discharge Team is seeking a hardworking, dynamic, proactive, and friendly nurse to join their liaison team in the Band 6 role.
The Aberdeenshire Discharge Hub facilitates the safe and effective discharge of patients from Aberdeen Royal Infirmary to home, community hospital, or other care settings in a timely manner, ensuring appropriate length of stay in the acute hospital setting. The post holder will collaborate with Multidisciplinary Teams and Care Management colleagues to determine discharge needs, proactively moving patients to the most suitable care environment using specialist knowledge, networking skills, and effective communication. Responsibilities include educating, advising, supporting staff, and facilitating the efficient movement of appropriate patients. The post also involves supporting complex discharges to long-term care facilities and maintaining communication between care management and prospective care homes.
The discharge hub operates a 7-day service from 0800 to 1600 hours.
The ideal candidate will demonstrate excellent leadership, confident communication, and effective organizational skills. Evidence of personal development is expected. You should have effective communication skills and the ability to work independently and as part of a team. We seek an individual with enthusiasm and commitment who will enjoy the challenges of this role.
Essential requirements include general medical or community nursing experience within NHSG.
