Job Profile Summary
Role:
The RN, Nights and Weekends provides after-hours field visits to patients, families, and community partners. Establishes and maintains positive relationships with team members, patients/families, and referral resources.

Qualifications:

1. Current license as RN in the state where the employee will be working
2. Minimum of one (1) year nursing experience; hospice or hospital experience preferred
3. Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
4. Ability to effectively communicate in English (verbal and written); bi-lingual in Spanish preferred
5. Mobile Driver - Valid driver’s license and automobile insurance per Company policy
6. Reliable transportation to meet patients’ needs
7. Excellent critical decision making and problem-solving skills
8. Excellent clinical and assessment skills
9. Ability to use equipment with visual and auditory mechanisms
10. Excellent time management skills with the ability to prioritize demands to meet patient service standards and deadlines
11. Ability to effectively handle stressful situations professionally and calmly with exceptional customer service
12. Strong patient service skillsInformation seeking / probing skillsAccurate knowledge transferListening, understanding, and responding
13. Able to navigate in a fast-paced environment utilizing interpersonal skills during patient visits
14. Ability to work in a cooperative team environment

16. Skilled in computer operations; proficiency in MS Office software applications, on-line research and proven data entry and navigation skills
17. Available to work shifts to accommodate extended hours of operation as scheduled
18. Ability to perform sedentary work with frequent interruptions
19. Ability to manage the emotional stress of working with and caring for terminally ill patients and their families
20. Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodations
21. Active BLS for healthcare professionals from the American Heart Association or Red Cross.

Some locations may require:

22. Completes clinical admissions for all Company services including, but not limited to:Explains services to potential patients/families and addresses questions regarding patient fears, physical limitations, while putting the patient/family at ease; presents services in an empathetic and compassionate manner Identifies admission objections, concerns or other entry barriers and provides responsesEducates patient/family on admission requirements and addresses immediate needsCompletes informed consents and initial assessment according to regulations and Company policyProvides information to Physicians and initiates Plan of Care to address patient’s immediate needsInitiates skilled nursing interventions to prevent complications, alleviate symptoms and maximize physical and emotional comfortObtains Physician ordersCompletes documentation per Company policy
23. Acts as the Company representative at assigned facilities while facilitating referrals to all service lines; works closely with referring hospitals, physicians, facilities, patients, families, and the general public.
24. Communicates frequently with Admissions to ensure timely follow-up. When assignment is complete, informs Admissions and prepares for next assignment.
25. Provides all necessary clinical communication timely using SBAR.
26. Discusses any potential needs with after-hours staff.
27. Obtains consents for services and arranges safe discharge in an efficient manner for referred and eligible patients while meeting productivity expectations.
28. Obtains medical record information for the referral.
29. Develops strong relationships with case managers, physicians, etc. at facilities.
30. Works with Professional Relations Representatives (PRRs) to provide education on all available services.
31. Provides care for any GIP patients in a hospital including daily visits, professional management, documentation and efficient discharge.

Job Responsibilities:

46. Makes after-hours visits to patients in their home, assisted living facility, nursing home, or hospital, anywhere within the assigned affiliate(s) territory. Provides compassionate customer service to patients and families.
47. Assesses patient/family and identifies immediate needs and appropriate nursing interventions by phone and/or during visit.
48. Provides and manages direct care to patients and families as part of Interdisciplinary Group (IDG), incorporating psychosocial, spiritual, cultural, physical and biological components, and appropriate nursing intervention and follow-up.
49. Demonstrates clinical skills regarding case management, application of company policy and procedure, physical assessment, intravenous therapies, pain medication titration, symptom management, and patient/family relationships.
50. Prioritizes, consults, refers, and collaborates with other professionals to promote quality and continuity of patient/family care.
51. Educates patient, family, caregivers and other health professionals about disease process and decline, palliative interventions, care giving, dying process, and safety practices.
52. Reports changes in the patient’s condition to appropriate members of the IDG or other health professionals.
53. Attends patient deaths; prepares body for transport; oversees disposal of medications; and provides bereavement support.
54. Completes documentation per policy and procedure.
55. During times of emergencies (i.e. Hurricanes, etc.), the RN, Nights and Weekends may be required to report to work at a location designated by the company, to ensure continuity of services. This may include reporting to work ahead of your scheduled date/time due to planned lock down of unit, and staying overnight(s) based on duration of emergency.

56. Perform other duties as assigned.

Compensation Pay Range:

$65,996.22 - $98,994.33

This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as on-going compliance with the Drug-Free Workplace Policy.
