Hospital at Home is expanding. Are you a Registered Nurse (RN) seeking a new opportunity? We have community nurse positions available within the Hospital at Home service, covering the county of Somerset. Bases include Taunton, Bridgwater, West Mendip, and Yeovil.
Hospital at Home enables people to receive acute care and treatments in their home surroundings with support from a team of health and care professionals. Responsibilities include attending MDT and huddles to discuss patient caseloads and plan interventions, conducting autonomous assessments and treatment plans, managing patient expectations, working within scope of practice, and seeking learning opportunities within the team. You will communicate openly and honestly with patients, families, and stakeholders, prioritize your time, support team efforts, and build relationships with neighborhood teams to support patients remaining at home. You will also participate in service development, identify your training needs, and support training for your team and other teams within the directorate.

Our service covers urban and rural areas of West Somerset, from the Quantock Hills to Exmoor. These positions are dayshifts, typically 08:00 – 20:00, including weekends, with no night shifts. We offer flexible working solutions where possible to promote work-life balance, and part-time hours will be considered. You must be a car driver with your own vehicle to visit patients. We provide a generous mileage allowance to cover NHS business use, including mileage and depreciation.
