Cardiac Rehabilitation Specialist Nurse

An exciting opportunity has arisen for a Cardiac Rehabilitation Specialist Nurse to work as part of the cardiology nurse specialist team. The post holder will care for patients referred to the Cardiac rehabilitation team in both primary and secondary care, working within the multi‑disciplinary team.
Job Responsibilities
* Act as a key member of the multi‑disciplinary team, actively participating and working within the guidelines of the relevant MDT.
* Coordinate the care of patients throughout their pathway, from diagnosis through treatment and follow up care.
* Provide appropriate written information for patients, relatives and hospital staff covering the relevant condition and its treatment.
* Provide psychological, social and cultural support to patients diagnosed with cardiology related conditions.
Qualifications and Experience
* Proven experience of cardiology and of caring for cardiology patients.
* Enthusiastic, committed and able to work well and flexibly within a multi‑disciplinary team.
Additional Information
Weekly working hours: 30 per week.
