Job Summary
We are looking for an experienced Health Care Assistant with enhanced skills to join our Frailty SDEC/Loddon nursing team. The role involves delivering clinical and therapeutic care to frailty patients within the Band 3 Health Care Assistant remit.
Key Responsibilities
* Deliver clinical and therapeutic care to patients as required within set protocols.
* Plan and deliver high standards of individualized personal care in accordance with Essence of Care benchmarks and Trust policies.
* Communicate effectively at all times within the team, with other health care professionals and with the patient, family, carers and all visitors.
* Accurately and responsibly carry out the instructions of the registered members of the multidisciplinary team.
* Under the delegation of the Registered Practitioner, undertake clinical skills as generally agreed to be appropriate for the role, such as wound care, taking blood, ECG, monitoring vitals, etc.
Qualifications and Experience
* Significant experience in a Healthcare Support Worker role, with enhanced skills particularly within the frailty team.
* Competency assessed for required clinical skills.
