Senior Service Co-ordinator – Primary Care Location: York
Contract: Permanent - 37.5 Hours Per Week
We are seeking a highly organised and proactive Senior Service Co-ordinator to join our Primary Care Team.
This pivotal role ensures the smooth running of services across our primary care service, supporting the delivery of safe, efficient, and patient-centred care. Working closely with GPs, practice managers, and a range of healthcare professionals, you will help shape services that meet the needs of our local community.

Oversee and co-ordinate day-to-day service delivery across practices within the service.
Line-manage and support administrative and co-ordination staff to ensure smooth service operation.
Develop and maintain effective systems for patient flow, referrals, scheduling, and reporting.
Act as a central point of contact for staff, patients, and partner organisations.
Support delivery of national and local initiatives, such as enhanced access, frailty pathways, vaccination programmes, and long-term condition management.
Monitor service performance, analyse data, and prepare reports for the leadership team.
Identify and implement improvements to enhance patient experience and operational efficiency.
Ensure compliance with information governance, safeguarding, and health & safety policies.
