Overview

The post holder will be a SCPHN Qualified School Nurse, who will provide specialist advice and evidence-based practice to clients and schools in Enfield to ensure that children and young people with long-term health conditions are well supported and have a safe environment at school. This role influences the work of the wider school nursing workforce and supports service development for long-term conditions.


Responsibilities

* Lead on the implementation of the annual work plan for long-term conditions.
* Identify gaps, good practice and opportunities for service development to be included in the annual workplan for long-term conditions.
* Participate in partnership meetings to improve Enfield’s response to long-term conditions and attend meetings and forums to promote initiatives in the annual plan.
* Initially focus on improving the management of childhood asthma by leading the implementation of an Asthma Friendly Schools initiative in Enfield to support children, young people, families and schools and reduce school days missed due to asthma.
* Develop Enfield School Nursing Service’s approach to other long-term conditions (e.g., allergies, epilepsy, eczema, sickle cell and diabetes).
* Support the 0-19 Service Team to achieve Asthma Friendly Accreditation in Enfield Schools.
* Work in partnership with the 0-19 School Nursing and Health Visiting Teams, service users and stakeholders to promote health and well-being of children and young people aged 5-19 and their families in the school environment, using evidence-based practice and guidelines (NICE, Government directives, NHS Plans).
* Identify and monitor health needs of the population and facilitate evidence-based care.
* Ensure effective delegation, workload management, and monitoring of care standards with leadership responsibilities.


Linkages and Practice

The main link with schools and family hubs is to promote, advise and guide current best practice and NICE guidelines, including Asthma Friendly Schools and Sickle Cell guidance.


Team and Workplan

An annual work plan for long-term conditions will be agreed between the Service Lead for School Nursing and the School Nurse for Long Term Conditions to inform the workstreams for the year ahead. This role works within the 0-19 North Mid community service, supporting families and young people via the Healthy Child Programme with agile working in the London Borough of Enfield.


Requirements

* Qualified School Nurse with SCPHN (School Nursing).
* Experience leading on long-term condition management and asthma-friendly initiatives.
* Ability to work in partnership with 0-19 School Nursing and Health Visiting Teams, service users and stakeholders.
* Evidence-based practice knowledge, NICE guidelines, and relevant NHS/Government directives.
