Clinical Lead Community Nursing

Join to apply for the Clinical Lead Community Nursing role at Cheshire & Wirral Partnership NHS Foundation Trust.

We are looking to recruit an enthusiastic Clinical Lead to join Princeway Community Care Team in the Frodsham area. You will be joining a multidisciplinary team including but not limited to Team Manager, Enhanced Clinical Practitioner, Clinical Leads, Community Nurses, AHP's and many other specialist professionals who visit the team.

We are looking for someone who is enthusiastic, self‑motivated and provides confident support and clinical leadership to clinicians while working in collaboration with other professionals. They will have excellent communication skills and the ability to work with patients and their families/carers, and when necessary, act to safeguard individuals who are at risk from harm and abuse.

This post is part time, 22.5 hours per week. The service runs between 8am and 8pm, seven days a week, and there will be an expectation that you will participate on the rota until 8pm.

Shortlisting for this vacancy is due to commence on 21/12/2025 and the proposed interview date is 31/12/2025.


Responsibilities

* Provide a highly visible clinical profile with leadership and clinical oversight of a designated community caseload.
* Work collaboratively with the wider health, social care and third sector partner agencies to meet the needs of patients.
* Ensure patients receive the right care, by the right person, at the right time and the right place.
* Place the patient at the centre of all decisions, reinforcing the concept of a more streamlined patient journey, with minimal avoidable delays and an emphasis on people achieving independence while improving quality of care.
* Provide direction within the caseload in consideration of the impact of current health and social care agendas and local initiatives.
* Be aware of local public health community profiles and how this influences the required care for the population.
* Based on community profiles, understand the skills required from the Community Care Team.
* Focus on the delivery of person‑centred care.
* Provide effective, evidence‑based care to a defined population through case load management and monitoring of care standards.
* Provide clinical leadership.

Cheshire and Wirral Partnership (CWP) provides health and care services for a population of over one million people, including mental health, learning disability, community physical health and all‑age disability care, as well as the provision of three GP surgeries in Cheshire.
