 
        
        Role Overview
The post holder will be an active member of the multi‑disciplinary team (MDT), providing routine personal care activities for patients, such as bathing, toileting, dressing, and supporting meals and refreshments. They will also assist patients with personal appearance and basic hygiene, including routine nail and hair care. The role involves collecting and updating basic patient information (e.g., age, weight, height, fluid and nutrition input/output) in patient records and reporting any concerns to the appropriate registered professional.
Key Responsibilities
 * Perform personal care activities, including bathing, toileting, dressing and continence support.
 * Support patients with nutrition and hydration needs, assisting with fluid/nutritional intake and completing input/output records.
 * Assist with patient transfers using hoists, wheelchairs and other equipment, ensuring safe positioning.
 * Maintain accurate and timely record keeping, updating electronic or paper‑based systems and ensuring entries are counter‑signed by a registered professional.
 * Escalate incidents promptly, report and record any incidents such as aggression or distressed behaviour, and support risk assessment by senior staff.
 * Communicate effectively and respectfully with patients, families, carers, colleagues and other professionals, listening and considering barriers to communication.
 * Act as an advocate for the patient, respecting wishes, dignity and beliefs.
 * Promote and maintain the Trust’s values, code of conduct and professional reputation through courteous, compassionate and safe care.
Qualifications & Training
 * Level 1 maths and English functional skills.
 * Care Certificate completed within the first 6 months of employment (if not already acquired).
 * Successful completion of DBS clearance (cost paid via salary deduction) and a 6‑month probationary period for new employees.
Employment Details
 * Working hours are up to full time (37.5 hours per week); part‑time applications will be considered.
 * Potential to work across multiple sites within the University Hospitals Tees group.
 * Health & Care Worker or Skilled Worker visa sponsorship available for eligible candidates.
 * Band 2 role.
