Overview
Preparing for Adulthood (PfA) Coordinators manage the Education, Health and Care (EHC) Plan process to ensure that young people with SEND aged 14-25 receive appropriate assessments, services, and provision. Coordinators are responsible for a caseload as defined by the Service Manager and coordinate the entire process with families to create an EHC Plan.
Responsibilities
* Present requests to the appropriate Panels and coordinate assessments from other agencies.
* Draft and finalise the EHC Plan with families.
* Act as a professional exemplar with a "can do" attitude and continuously improve customer satisfaction levels for the service.
* Undertake duties efficiently and timely, reach appropriate placement decisions, and manage Phase Transfer and transitions through liaison with Social Care and Health services, schools/colleges, and parents/carers.
* Work closely with service providers and families to ensure provision in EHC Plans is delivered and take action where it is not.
* Manage processes including Annual Reviews and carry forward requests to change parts of the EHCP.
* Lead Annual Reviews as required and input into monitoring systems for the management of all Review processes.
* Be aware of requirements for the participation of young people over compulsory school age and advise professionals on matters arising from mental capacity issues.
* Ensure young people have an appropriate transition plan in line with statutory requirements; stay up to date with good practice on transitions for young people with SEND.
Requirements / Qualifications
* Experience with Education, Health and Care Plans (EHC) and PfA coordination.
* Knowledge of statutory transition requirements and mental capacity considerations.
* Ability to liaison with Social Care, Health services, schools/colleges, and families to coordinate assessments and reviews.
