1 Month Contract With A Local Authority
Job Purpose:
* As a Social Worker, you will provide professional support and intervention to adults with care and support needs, helping them maintain independence, safeguard their well-being, and improve their quality of life. You will work in partnership with individuals, families, carers, and multi-agency teams to deliver effective, person-centred outcomes.
Key Responsibilities:
* Undertake strengths-based assessments of individuals’ care and support needs in line with the Care Act 2014.
* Develop, implement, and review care and support plans tailored to individual needs and goals.
* Safeguard adults at risk by identifying and responding to concerns in accordance with safeguarding procedures.
* Work collaboratively with health, housing, voluntary sector, and community services to ensure holistic support.
* Provide information, advice, and advocacy to enable informed decision-making.
* Maintain accurate and timely case records using electronic case management systems.
* Participate in multidisciplinary meetings, case conferences, and reviews.
* Manage a varied caseload, prioritising effectively to meet statutory responsibilities and deadlines.
* Promote and uphold the principles of dignity, choice, and independence.
* Keep up to date with legislation, policy, and best practice in adult social care.
Requirements:
* Degree in Social Work (or equivalent)
* Registration with Social Work England
* Understanding of the Care Act 2014, Mental Capacity Act 2005, and safeguarding frameworks
* Experience working with adults in a social care or health setting
* Strong communication, assessment, and case management skills
* Ability to work independently and as part of a team
* Commitment to anti-discriminatory practice and service user empowerment
