Job Summary
West Kent Primary Care (WKPC) is seeking to employ an Advanced Nurse Practitioner (ANP) on behalf of Mote & Wallis Primary Care Network (PCN) to work across its three member practices. This role supports The Mote Medical Practice and Wallis Avenue Surgery, with a combined patient population of around 17,000. The post holder will work across three sites alongside the PCN Lead Pharmacist and PCN Pharmacy Technician. The PCN also provides MSK and Children’s Social Prescribing services.

Responsibilities
* Work as part of a multi-disciplinary team to deliver safe and high-quality care.
* Carry out clinical nursing practice at an advanced level using expert clinical skills and critical judgement.
* Assess, diagnose, plan, implement and evaluate treatment and care for patients with acute, long-term and complex health needs.
* Triage and treat patients, performing assessments and making referrals as necessary to other members of the primary and secondary health care teams.
* Interpret investigation findings to determine effective treatment plans and discuss with GPs when a second opinion is required.
* Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition, using both non-pharmacological and pharmacological treatments.
* Coordinate the planning and delivery of care, ensuring patients and carers or relatives are involved where required.
* Prescribe and review medication appropriate to patient needs and in accordance with the latest evidence-based practice and national protocols.
* Request diagnostic investigations and interpret results within agreed protocols (e.g., x‑ray, blood tests, scans).
* Assess, diagnose and treat patients in practice premises or at patient homes who require medical attention, or refer to appropriate agencies.
Experience
* Significant post‑registration experience.
* Recent primary or community nursing experience.
* Experience in nurse‑led management of minor illness, minor ailments and injuries.
* Experience in nurse‑led triage.
* Experience compiling protocols and clinical guidelines.
* Experience in clinical leadership.
* Experience in audit and research.
* Experience in nurse prescribing.

* Salary: £53,755 to £60,504 per year (pro rata may apply).
