A vacancy has arisen to join our excellent nursing team because of partial retirement. You will join a team of experienced practice nurses and you will work alongside another nurse most days albeit at different surgeries. You will supervise and work with an experienced Health Care Assistant, who does basic dressings under your delegation. They also take bloods, anticoagulation, ECGs, health checks, adult immunisations under a PSD.

You will run your own clinic and will see a variety of patients for differing reasons during each clinic at set intervals so you need to be able to work independently .

Applications are welcome from candidates with practice nursing experience or other nurses with substantial post registration qualification considering a career change. Candidates must demonstrate the commitment and development potential for the role specifically chronic disease management.

Duties will include chronic disease management, cytology, children and adult vaccinations, contraception, dressings and general treatment room duties.

Chronic disease management experience would be beneficial. We will support training for the right candidate.

Salary will be dependent on qualifications and experience

You will be required to work at both surgeries and available to help cover leave absences.

1 day/9 hours per week


Main duties of the job

Working one day per week you will independently run a prebooked morning and afternoon clinics seeing a variety of patients with different presentations making clinical judgements and decisions. You will see urgent 'on the day' patients in designated slots.

You will assess and manage the care of practice patients to include treatment, preventative care, screening & patient education. To recognise own training needs and limiting your actions to those that you feel competent to undertake.To develop and adhere to clinical practice protocols. To deliver evidence based practice for patients presenting with long term conditions.

Appointment times are 15-30 minutes depending on booking reason. Working 08.00 - 17.30 with admin time in the middle of the day and a 30 minute lunchbreak.

You will agree wound treatment plans and delegate less complex dressings to Health Care Assistants.

You will support the practice in promoting healthy behaviours and making every contact count.

You will ensure the timely recall of chronic disease patients and work with the admin support team.

Rural practice with 2 surgeries and 8200 patients in beautiful South West Anglesey. Llanfairpwll Health Centre & Meddygfa Penbryn Surgery. 7 GPs 1 ANP, experienced practice nurses, health care assistants and a well established admin team. EMIS clinical system. Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs

Fully competent in all Health Care Assistant tasks e.g. venepuncture, ECGs, INR monitoring, BPs, urinalysis, new patient medicals, basic dressings, ABPM, chaperoning, assisting with minor surgery etc.

Removal of sutures

BPs and lifestyle advice

Cytology

Basic family planning/contraception advice/sexual health

Advice and refer STIs

Breast Awareness

Swabs

Adult Immunisations eg. tetanus, flu, pneumococcal etc

Travel advice and vaccinations

Ear care and irrigation

Administration of nebulised medication

CHD reviews & self management advice

Recognise signs of mental health problems & when to refer

Childhood immunisations

Long term condition management of 2 specialist areas Diabetes and respiratory, provide advice, comprehensive review including the alteration of medication dosages as required and medication reviews, provide patients with their own self management plan

Take a lead role in the practices programmes of care for patients with long term conditions, cytology, immunisations

All aspects of Family Planning including initiate LARC/Depo Provera Treatment

Triage & Minor illness, assess and provide treatment according to practice protocols

Administrative

Ordering treatment room stock/vaccinations, to maintain adequate stock levels with due regard to the practice drug budget, practice resources and reimbursement

When and how to liaise with specialist nurses and outside agencies

Maintaining effective recall systems

In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential

Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data

Health & safety & risk :

The post-holder will implement and lead on a full range of promotion and management of their own and others health and safety and infection control as defined in the practice Health & Safety policy, the practice Health & Safety manual, and the practice Infection Control policy and published procedures. This will include (but will not be limited to):

Using personal security systems within the workplace according to practice guidelines

Awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements, and good practice guidelines

Responsible for the correct and safe management of the specimens process including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements

Management and maintenance of Personal Protective Equipment (PPE) for the practice including provision, ordering, availability and ongoing correct usage by staff

Responsible for hand hygiene across the practice

Ownership of infection control and clinically based patient care protocols, and implementation of those protocols across the practice

Active observation of current working practices across the practice in relation to infection control, cleanliness and related activities, ensuring that procedures are followed and weaknesses / training needs are identified, escalating issues as appropriate

Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks across clinical and patient process

Making effective use of training to update knowledge and skills, and initiate and manage the training of others across the full range of infection control and patient processes

Monitoring practice facilities and equipment in relation to infection control, ensuring that provision of hand cleansing facilities, wipes etc are sufficient to ensure a good clinical working environment. Lack of facilities to be escalated as appropriate.

Safe management of sharps procedures including training, use, storage and disposal

Using appropriate infection control procedures, maintaining work areas in a tidy, clean and sterile, and safe way, free from hazards. Initiation of remedial / corrective action where needed or escalation to responsible management

Actively identifying, reporting, and correction of health and safety hazards and infection hazards immediately when recognised

Keeping own work areas and general / patient areas generally clean, sterile, identifying issues and hazards / risks in relation to other work areas within the business, and assuming responsibility in the maintenance of general standards of cleanliness across the business in consultation (where appropriate) with other sector managers

Undertaking periodic infection control training (minimum twice annually)

Routine management of team areas, and maintenance of work space standards

Waste management including collection, handling, segregation, container management, storage and collection

Spillage control procedures, management and training

Decontamination control procedures, management and training

Clinical equipment maintenance

Maintenance of sterile environments

Maintaining the cold chain

Equipment and in date emergency drugs to treat anaphylaxis

Attend monthly practice nurse team meetings (min 8 p.a.). Participate in audit where appropriate. Health promotion activities.Participate in the annual flu campaign and other Public Health campaigns
