We have an exciting opportunity to recruit an Intellectual disability nurse to our Primary care liaison Team.
We are looking for a motivated and enthusiastic RNLD who is passionate about supporting reasonable adjustments for patients with intellectual disabilities in the community.
We welcome applicants who are interested in supporting people with intellectual disability to enable them to have their physical health needs met within a primary care setting.
Responsibilities
* Work with individuals to facilitate their access to primary health care services and liaise with other health professionals within multi‑agency teams to advocate for service users.
* Work with local GP practices to identify patients eligible for an annual health check, flag them and encourage them to have an annual check.
* Deliver training around the Enhanced Service around health checks to primary care colleagues.
* Work with the national screening programmes to support them to meet the needs of the local intellectual disability population by breaking down barriers to support access to screening.
* Commit to improving the life chances of people with intellectual disabilities.
