Job Purpose
An exciting opportunity for an experienced Preparing for Adulthood (PfA) EHCP Coordinator to join a dynamic team in supporting young people with special educational needs and disabilities (SEND), ensuring close collaboration with families and professionals during the Education, Health, and Care Plan (EHCP) process. This hybrid role requires office presence on Mondays and Tuesdays and is instrumental in enabling smooth transitions for young people from age 14 through to adulthood.
Key Responsibilities
* Manage a caseload of EHCPs for young people aged 14-25, guiding families and professionals through the statutory assessment and planning process.
* Present assessment requests to Panels and coordinate input from education, health, and social care agencies.
* Draft, review, and finalise EHCPs, ensuring all voices, including families and young people, are properly represented.
* Oversee phase transfers and transitions, working closely with schools, colleges, and service providers to ensure provisions are met.
* Lead and coordinate annual review meetings, updating EHCPs in accordance with statutory requirements.
* Monitor provision delivery and take action where required to address gaps.
* Advise on participation requirements for post-compulsory schooling, including issues arising from mental capacity.
* Liaise with a range of stakeholders, ensuring accountability and quality in EHCP delivery.
* Use digital tools (e.g. Microsoft, Google) for the timely administration and record-keeping of casework.
* Remain up-to-date with best practice around SEND transitions and preparing for adulthood.
Person Specification
Qualifications & Education
* Educated to degree level or equivalent experience in education, children's services, social work, disability, psychology, or related field – Essential.
Experience
* Relevant experience and detailed knowledge of SEND law and the SEND Code of Practice, especially around transitions and preparing for adulthood – Essential.
* Experience in multi-agency working, especially in partnership with adult social care, vocational pathways, and supporting young people who are NEET – Essential.
* Track record of excellent written communication, including drafting high-quality plans and documentation.
Knowledge, Skills & Abilities
* Strong organisational skills: able to manage and prioritise a varied caseload, meeting statutory deadlines.
* Excellent interpersonal and customer service skills: able to communicate complex information clearly and sensitively.
* Ability to work autonomously, exercise initiative, and problem-solve challenging cases.
* Awareness of safeguarding issues, equality, diversity, and financial accountability within public services.
* Passionate about supporting young people to live safe, happy, and successful lives through collaborative practice.
* Digital literacy in using relevant hardware and software for case management.

* The role will require regular office attendance on Mondays and Tuesdays; no parking is available on site.
