Care Coordinator – upto 32 hours per week, part-time. Join the North Kerrier East Primary Care Network (PCN) as part of the Health and Well-being team. This role supports people to navigate and signpost to other key services, in a co-located, inclusive, supportive, and innovative environment.
Salary: Depending on experience up to £27,485. Contract: Fixed term, 12 months. Location: Redruth, Cornwall (C/O Leatside Health Centre, Forth Noweth, Chapel Street, Redruth, Cornwall, TR15 1AU).

Responsibilities
* Work as part of the PCN health and well-being team, coordinating care between GPs, practice nurses, community matrons, volunteer teams, physiotherapists, mental health practitioners, and health and well-being coaches
* Record all patient contacts and work on the clinical system against the patient record
* Work with individual patients, their families and carers, using a holistic approach, to identify goals for care, and agree a personalised care and support plan with signposting to other services
* Support delivery of care plans by coordinating input from a range of professionals and services, helping patients and carers navigate health and social care services
* Help patients manage their needs by answering queries, being a first point of contact across the PCN, and by making and managing appointments
* Support patients to utilise decision aids and ensure access to good quality information for shared decision-making
* Use tools such as patient health questionnaires when engaging with patients
* Help patients access self-management education courses, peer support or other interventions to improve health and wellbeing
* Undertake regular reviews of personalised care and support plans
* Develop personalised care plans in line with national best practice
* Work with patients over the phone, in person at the practice, or, for housebound patients, carry out home visits as necessary
* Use practice level reports to identify suitable patient cohorts for personalised care and support, supporting with specialist clinics
* Provide accurate and timely data to support audit and monitoring, and any required data returns
* Keep accurate records of contacts with patients and carers in clinical systems and care plans
* Follow handover procedures between settings and ensure care packages are set up
* Organise meetings to support care planning, circulate required information, and follow up actions in a timely way
* Develop strong relationships with key organisations involved in patient care planning
* General administration duties to support the PCN Business Manager and team

Person Specification
Essential
* Experience in health, social care, third sector or information and support services with direct contact with people, families and professionals
* Customer care experience
* Significant experience in organisation, planning and coordinating skills
* Ability to prioritise own workload, use initiative and meet deadlines
* Able to extract, analyse and interpret data
* Excellent communication skills, verbal and written, with ability to adjust style to the audience
* Proven ability to use computers and Microsoft Office and IT systems
* Driving licence and access to transport
Desirable
* Higher level qualification such as NVQ in health and social care Level 3
* Qualification relevant to health or social care or children
* IT qualifications
* Experience of working in health services
* Experience of working as a care coordinator or social prescriber
* Experience or training in personalised care and support planning
* Experience of working with elderly, children or vulnerable people, complying with best practice and relevant legislation
