Due to additional investment into the service we have a newly created band 5 post. The successful candidate will work across Trafford Community supporting patients in their own home, residential/nursing home, and clinic settings. This post is to cover adult services. Our caseload includes support for patients with a variety of clinical conditions for example cancer, dementia, respiratory diseases, gastrointestinal conditions and age‑related illness. We work closely with our community colleagues across the MDT to provide high quality patient care.
Strong communication and time‑management skills are essential to allow you to independently manage a caseload, but we will support you via clinical supervision and from other dietitians and team leads. Team‑working skills are also key, and we take a whole‑team approach to supporting and growing the dietitians in the team.
Responsibilities
* Carry a general clinical caseload involving assessments and treatment plans for patients in their own home, residential/nursing home or clinic setting.
* Give clinical dietetic advice as appropriate to support service planning and delivery.
* Be involved with dietetic student training and support the development of the next generation of the dietetic workforce.
