Overview

The Outreach practitioner role is to work in an outreach service within the wider health and justice service. The service will support vulnerable children and adults who have entered the criminal justice system, including adults and children released from police custody with additional health needs. The team provides support for up to 12 weeks in the community to reduce risks around reoffending and health vulnerabilities. The role involves developing pathways of support for children and young people and establishing connections with partner agencies to provide timely interventions. This is a new development and offers the opportunity to help create new pathways of support.


Responsibilities

* Develop and oversee triage and referral pathways for adults, children and young people, in collaboration with the team leader.
* Work within the outreach service to support adults and children and young people referred after contact with criminal justice services.
* Develop pathways of support in partnership with agencies, thinking creatively to improve outcomes and reduce reoffending risk.
* Promote and support self-determination, choice and autonomy in care planning and delivery.
* Provide written reports for Crown or Magistrates Courts as required and attend/advise as needed.
* Contribute to setting quality standards, auditing, monitoring and reviewing practice in line with clinical guidance and policy.
* Assess individual care needs, refer to appropriate agencies, and contribute to robust risk assessments and risk management plans.
* Undertake outcome measurement to review the effectiveness of the service and participate in clinical governance at team level.
* Ensure the aims and objectives of the service are fulfilled and identify factors that may inhibit progress.

Qualifications

Essential

* Registered Mental Health Nurse / Learning Disability Nurse / General Nurse on NMC Register.
* Registered Social Worker.
* Post-registration experience with formal courses and training at diploma/degree level.

Desirable

* Leadership qualification.
* BSc in nursing or equivalent.
* Psychological intervention qualifications (e.g., EMDR, CBT, CBTp, Family Interventions, CAT, DBT, Structured Clinical Management).


Experience

Essential

* Extensive post-registration experience with clients experiencing mental health difficulties.
* Experience with clients who have difficulty engaging with services and complex needs.
* Experience in community settings with autonomous decision-making.
* Experience coordinating care in community settings and working with multi-disciplinary/multi-agency teams.
* Experience in clinical audit.

Desirable

* Lived experience of the criminal justice system or related vulnerabilities.


Skills, Knowledge & Abilities

* Knowledge of Mental Health NICE Guidelines and the NMC Code of Conduct.
* Understanding of Care Programme Approach and Community Care Act.
* Commitment to Equality and anti-discriminatory practice.
* Knowledge of statutory, independent and third-sector services for adults with mental ill health.
* Ability to assess, plan and evaluate care, including risk assessment and management.
* Ability to interpret physical health indicators and provide health promotion advice (weight, BMI, blood pressure, smoking, alcohol, drugs).
* Knowledge of Mental Health Act, Mental Capacity Act and DOLS.
* Excellent interpersonal skills and ability to work collaboratively with service users, carers, and other providers.

* Willingness and ability to travel for community work with appropriate vehicle insurance if applicable.
