About the Role
We are seeking an Inclusion Officer to support children and young people (025) with Special Educational Needs and Disabilities (SEND). You will play a key role in carrying out statutory requirements under the Children and Families Act 2014 and the SEND Code of Practice 2015, ensuring that assessments, Education Health and Care (EHC) plans, and reviews are timely, effective, and outcome-focused.
This is a varied and rewarding role where you will work closely with parents, carers, schools, health professionals and other agencies to improve outcomes and promote inclusive practice.
Key Responsibilities
1. Case manage assessments and reviews for a designated caseload in line with legislation and statutory timeframes.
2. Ensure high-quality EHC plans are written, maintained, and reviewed annually.
3. Liaise with families, education providers, and health and care services to ensure coordinated planning and provision.
4. Advise schools and other education providers on statutory duties and inclusive practice.
5. Contribute to decision-making panels regarding specialist provision.
6. Support transitions into adulthood, ensuring effective preparation for adult life.
