The Community Rehabilitation Team (CRT) West supports people living with complex mental health problems to achieve their goals, along with gaining the skills and confidence to live as independently as possible. CRT West works as an MDT, discussing, formulating and delivering individualised plans. We are looking to recruit a dynamic, enthusiastic and experienced Social Worker to the team on a full time basis. You will need to work creatively to deliver a high standard of care that optimises a person's own health, wellbeing and recovery. We work in a responsive way to provide stability in a person's life and help them to develop meaningful occupation alongside managing the symptoms of their mental illness. The role of social worker in the team will lead on all aspects of social care, and will require experience, or keen interest, in working within a specialist service. Knowledge and application of the Care Act, Mental Capacity Act and Mental Health Act is essential to the role.
Qualifications
* Have a degree in social work/Occupational Therapy or equivalent
* Be registered with the Health and Care Professions Council (HCPC)
* Be able to fulfil the travel requirements of the post
* Knowledge and application of the Care Act, Mental Capacity Act and Mental Health Act is essential
