ForestMedical Group are looking to recruit a Practice Nurse to join our friendlyteam, due to our rapidly growing patient list size and expansion of the Group. Theideal candidate will already possess experience of Practice Nursing and atleast one complex area, ideally Diabetes and want to seek further opportunitiesto develop and build on an already established career.


Main duties of the job

As a Practice Nurse, youll provide high-quality nursing care, including managing long-term conditions, delivering vaccination programmes, supporting womens health clinics, and offering general health advice. Youll also take part in screening, wound care, minor procedures, and health promotion, while working closely with our multidisciplinary team to ensure the best outcomes for our patients.

Job responsibilities

JobDescription

Responsibilities Include:

Providingassessment, screening treatment services and health education advice:

Dietary advice

Carry out specific health screening to thepractice population including cervical cytology.

Assist in the provision of care to thosepatients with chronic diseases operating specific disease clinics, for exampleasthma, diabetes, and coronary heart disease etc.

Screen patients for disease risk factors andearly diagnosis of illness and help monitor a recall system for patients.

Advise patients on general health care andminor ailments with referral to the general practitioner as necessary.

Give immunisations and vaccinations inaccordance with the Practices group directive and give travel vaccinationsand advice for travellers.

Give advice on family planning and recommendappropriate contraceptives.

Carry out routine blood taking and bloodpressure monitoring.

Give diet and healthy lifestyle advice.

Chaperoning and assisting patients whereappropriate who are being examined by another clinician.

Provide wound care and Doppler assessment.

Carry out ECGs tests.

Carry out INR testing using Coagucheck and INRStar

Urinalysis

Health screening

Ear syringing

Dressings

Removal of sutures / clips

Baby immunisations

Assisting with the fitting of IUCD / IUS

Organise and coordinate the provision ofnursing services for the practice to include the delegation of duties to othermembers of the practice nurse team as appropriate.

Provide general and specific health screeningsto the practice patients (within agreed protocols) with referral to othermembers of the primary and secondary health care team as necessary.

Undertake the collection of pathologicalspecimens including intravenous blood samples, swabs etc. Perform investigatoryprocedures requested by the GPs.

Minor Operations

Management of the theatre and recovery suitefor minor operations including maintenance of equipment and stock, adequatestaffing and ensuring appropriate items of service claims are made.

To assist the doctor to carry out minoroperations as required.

Report any problems to the Practice Manager

Supplies and equipment

Maintenance of equipment and stock relating topatient care

Meetings

It will be necessary to attend and contributeto various Practice meetings as requested. The only reason for not attendingwill be annual, study or sick leave.

Communication:

The post-holder should recognise the importance of effectivecommunication within the team and will strive to:

Communicate effectively with patients andcarers.

Communicate effectively with other teammembers.

As well as the nursing team there is a need towork closely with reception, office, and dispensary staff to ensure the smoothrunning of the practice, reporting any problems encountered to the relevantperson and ensuring that everyone is aware of the different roles within thenursing team.

There is also the need to establish andmaintain good liaison with other surgeries and agencies including secondarycare.

Administrative and ProfessionalResponsibilities

To work in partnership with generalpractitioners, the nurse practitioner, practice nurses and other professionalsto develop protocols, standards, and audit tools to facilitate effectivedelivery of care.

Participate in the administrative andprofessional responsibilities of the practice team.

Ensure collection and maintenance ofstatistical information, on the computer or manually, required for regular andad hoc reports and audits.

Demonstrate and maintain adequate knowledgeand compliance with all aspects of health and safety at work and COSHH.

Order vaccines and Stock, tidy and organiseTreatment Rooms and Stock Rooms.

Maintain records of Clinic attendance andservices provided.

Ensure accurate and legible notes of allconsultations and treatments are recorded in the patients notes.

Ensure the clinical computer system is kept upto date with accurate details recorded and amended.

Ensure appropriate items of service claims aremade accurately, reporting any problems to the Practice Administrator

Ensure accurate completion of all necessarydocumentation associated with patient health care and registration with thepractice.

Training and personal development

Training requirements will be monitored byyearly appraisal and will be in accordance with practice requirements. Personaldevelopment will be encouraged and supported by the Practice. It is theindividuals responsibility to remain up to date with recent developments.

Participate in the education and training ofstudents of all disciplines and the induction of all members of the practicestaff where appropriate.

Participate in developing and maintaining anin-house nurse-training programme reporting to the Practice Nurse Manager.

Maintain continued education by attendance atcourses and study days as deemed useful or necessary for professionaldevelopment ensuring PREP requirements are met.

If it is necessary to expand the role toinclude additional responsibilities, full training will be given.

Develop and maintain a Personal Learning Plan

Contribute and participate as required in anyresearch projects and audits within the practice.

Assist inthe Management, supervision, co-ordination and guidance of the nursing team

To assist in the management of the nursingservice in participation with the Practice Nurse Manager ensuring relevantinformation is disseminated to and from the nursing staff.

In participation with the Practice NurseManager contribute to yearly appraisals of all the nursing staff producing atraining programme

Ensure adequate nursing cover is providedwhile within the practice guidelines, keeping the Practice Nurse Managerinformed of any problems.

Participate in the Development and maintenanceof practice policies and protocols for all nursing activity.

To lead / assist in agreed clinical areasi.e., infection control, Asthma, Diabetes etc. in the practice to ensure allstaff are aware of current procedures.

Participate in the Planning, chairing, andrecording of management and training meetings for all nurses following thetraining programme.

Ensure agreed practice audits are carried out.

Ensure the General Practice Manager isinformed of any matters arising or problems / potential problems.

Ensure all surgery produced nursing leafletsare kept up to date.

Produce a 6 monthly nursing newsletter forpatients.

Health & Safety

Thepost-holder will assist in promoting and maintaining their own and othershealth, safety and security as defined in the practice health & safetypolicy, the practice health & safety manual, and the practice infectioncontrol policy and published procedures. This will include:

Using personal security systems within theworkplace according to practice guidelines

Identifying the risks involved in workactivities and undertaking such activities in a way that manages those risks

Making effective use of training to updateknowledge and skills

Using appropriate infection controlprocedures, maintaining work areas in a tidy and safe way, free from hazards

Actively reporting health and safety hazardsand infection hazards immediately when recognised

Keeping own work areas and general / patientareas generally clean, assisting in the maintenance of general standards ofcleanliness consistent with the scope of the job holders role

Undertaking periodic infection controltraining (minimum annually)

Demonstrate due regard for safeguarding andpromoting the welfare of children.

Person Specification


Qualifications

* To be a Registered General Nurse
* To have or to express a commitment in working towards a Practice Nurse Diploma or equivalent.
* To have at least three years nurse experience
* Experience of building and maintaining professional working relationships
* To work in accordance with UKCC Code of Conduct.
* Knowledge of current clinical evidence-based practice.
* Knowledge and understanding of clinical governance.
* Knowledge of current professional issues relating to nursing
* Ability to self-motivate, organise, and prioritise workload.
* Excellent writing and computer literacy skills
