Overview
Diabetes Specialist Practice Nurse role based in Gloucester with responsibilities across primary care nursing, patient consultations and clinical administration.
Responsibilities
* Primary care nurse duties including face to face consultations, video consultations, telephone consultations, e-consultations and queries.
* Clinical administration, and dealing with queries, paperwork and correspondence in a timely fashion.
* Provision of care for acute and long-term conditions (physical and mental) and contributing to the delivery of QOF and enhanced services.
* Provision of preventative care, including health promotion advice, vaccinations and screening.
* Provision of education to patients & management of Diabetes. Support of patients in developing self-management strategies and fostering patient engagement with the Practice.
* Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
* Recording clear and contemporaneous IT-based consultation notes to agreed standards; collecting data for audit purposes as per Practice protocols.
* Reviewing patients in the interests of quality improvement and safety (e.g., MRHA safety alerts and dementia reviews).
* Prescribing in accordance with locally agreed or national guidelines.
* Cover for other practice nurses through reciprocal sharing of correspondence, results and other administration.
* Contributing to patient safety by maintaining good infection control, thorough monitoring and maintenance of stock and equipment to include refrigeration, sterilising equipment and emergency equipment.
* Complying with the Nursing and Midwifery Council's Code and professional standards for nurses.
* Complying with CQC requirements for primary care and with training requirements of G DOC and of the NMC; complying with requirements for revalidation.
* Working conditions involve frequent, prolonged VDU use in a time-pressured environment with a high need for accuracy and attention to detail; exposure to distressing situations and written material; contact with body fluids (e.g., wound exudates; urine) in clinical practice.
* Infection control: undertaking training and following policies; safeguarding is everyone’s responsibility with appropriate safeguarding training and procedures in place.
* Equality and Diversity: support the equality, diversity and rights of patients, carers and colleagues; respect privacy and dignity.
* Quality and Patient Safety: maintain quality, participate in audits and significant event analyses, manage time and resources effectively, and work with other agencies to meet patient needs.
* Participating in handover and follow-up arrangements for patients; informing line manager of concerns about colleagues or complaints; participating in investigations as required.
* Regulated professionals to comply with regulators' guidance and report any regulatory investigations; inform line manager of any restrictions on practice.
* Personal/Professional Development: maintain mandatory training up to date and participate in annual performance reviews and ongoing development.
* Communication: recognise and address communication needs, including use of translators when required.
Site and Safety
* Health & Safety responsibilities and local site procedures; immunisation in line with Green Book and NHS recommendations unless medically exempt; ensure competency in own practice; comply with health and safety rules including sharps, waste and hazardous substances; report safety hazards to Practice Manager.
* Dress code and professional presentation; ensure car insurance for commuting and business use if home visits are part of the role.
* Security: be aware of stalking risks and do not disclose rota information outside the Practice; refer rota queries to Practice Manager or G DOC.
