We are seeking a motivated, confident and compassionate Social Care Team Lead to join our integrated multi‑professional team. This role is central to ensuring safe, timely, and effective care for people in crisis at home across East Sussex — helping prevent avoidable hospital admissions and supporting people to remain where they feel most comfortable.
Responsibilities
* Provide daily operational support to Team Leads and shift coordinators
* Oversee effective rostering and workforce deployment
* Lead and mentor staff through supervision, live observation and feedback
* Support training, education and competency assessment frameworks
* Drive continuous improvement, service development and project work
* Support complex case management and inter-professional working
* Ensure safe, high-quality care aligned with Trust policy and safeguarding standards
* Foster a positive, inclusive and collaborative team culture
About You
You’ll be an experienced and motivated leader with a strong grounding in adult health or social care, bringing both confidence and compassion to your work.
You Will Have
* A good general level of education, including English and Maths, and be educated to degree level in a relevant field or have equivalent knowledge and experience gained in practice.
* Leadership training or qualifications at Level 4 or Level 5.
You Will Bring
* At least 3 years’ experience working with adults in health or social care
* Experience supervising or managing staff, including performance management
* Strong communication skills and confidence in supporting complex situations
* Ability to lead calmly under pressure and navigate challenging conversations
* Commitment to high-quality care, learning culture, and continuous improvement
