Role Purpose The Occupational Therapist supports adults to maximise independence, wellbeing and functional ability across community, social care and rehabilitation services.

Key Responsibilities 1.
Functional & Clinical Assessment Conduct comprehensive functional assessments.
Assess mobility, self-care needs and daily living skills.

Intervention & Rehabilitation Develop personalised rehabilitation plans.
Deliver therapeutic programmes and interventions.
3.
Equipment & Adaptations Recommend assistive equipment and environmental modifications.
Liaise with equipment providers and housing teams.
4.
MDT Collaboration Work collaboratively with physiotherapists, nurses, social workers and others.
Participate in MDT meetings, discharge planning and case reviews.
5.
Documentation & Governance Maintain accurate clinical notes.
Support audits and service evaluation.
Person Specification Essential: HCPC registered OT.
Strong assessment, planning and communication skills.
Desirable: Experience in neuro, community rehab or social care.
