Detailed job description and main responsibilities Please see attached Job Description and Person Specification- Main Duties and Responsibilities: Deliver and participate in the clinical care of patients recognised as frail, working within Gold Standards, and performing a Comprehensive Geriatric Assessment. Priorities health needs and intervenes appropriately. To be professionally and legally accountable for all work undertaken to practice at an advanced level of professional autonomy and accountability that is within Trust policies and the Nursing and Midwifery Code of Conduct (2008) or Health and Care Professions Council (HCPC). Provide a level of advanced and comprehensive health assessment based on a specialist body of knowledge. To appropriately assess, examine, investigate, diagnose, and treat patients, resulting in the safe management and appropriate referral or discharge of patients with undifferentiated and undiagnosed presentations. Ensure patients are informed and consent is gained prior to treatment, investigation, and management as appropriate. When patients are recognised as being incapacitated to make decisions about their own health needs, to be able to make decisions in the patient's best interest to enhance their care and well-being. Take responsibility for the management of patients within own caseload and supervision/development of junior members of the multi-professional team. Can identify the need for appropriate diagnostic tests and interpret the results. Person specification Qualifications Essential criteria Registered Nurse with Degree Teaching/Mentorship Qualification Already enrolled on the Advanced Clinical Practice MSc pathway Non- medical prescribing qualification Desirable criteria Leadership Qualification. Teaching or education qualification. Dementia Qualification. Advanced Life support. Knowledge and Experience Essential criteria Working at an advanced level of nursing with the necessary experience as a qualified nurse. Experience in geriatric medicine / care of the elderly setting within acute or community settings. Knowledge and experience in both admission and discharge processes within the acute trust. Knowledge and experience of the Mental Capacity Act. Understanding of Governance and assurance systems and application to practice. Desirable criteria Worked in multiple care settings including acute and community care. Quality improvement experience. Experience in Audits and change management. Specialist Knowledge of geriatric medicine including dementia and safeguarding. Skills Essential criteria Models excellent clinical skills and is able to implement these into practice. Venepuncture & phlebotomy. Catheterisation Bladder scan ECG. Able to provide and receive highly complex, sensitive and contentious information. Demonstrates verbal, written & interpersonal skills. Able to use IT data bases computer literate, proven ability to use word, excel and email. Desirable criteria Ability to carry out a mental Capacity Assessment and Best interest Assessment.
