What will your responsibilities be?
* Required to complete standard annual reviews in Neighbourhoods, Learning Disability, and Mental Health teams.
* Focused on Care Act-compliant annual reviews of existing care and support plans.
* Hybrid working available, with presence required within respective teams as needed.
* Roles sit within core adult social care teams, aligned to service specialisms.
* Candidates must be able to work independently and manage review caseloads effectively.

Qualifications & Experience:
To be successful in this role you must have,
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK driving licence
* Post-qualified experience
* Degree level or equivalent in Social Work
