About the Role:
We are seeking an experienced and motivated Social Worker to join our Hospital Discharge Team in Bristol. This is a fast-paced, rewarding role where you will work closely with health professionals to support safe, timely discharges and deliver high-quality, person-centered interventions.
You will play a vital role in promoting independence, enabling self-directed support, and ensuring statutory duties are fulfilled in line with current legislation and best practice.
Key Responsibilities:
* Carry out statutory duties and responsibilities within relevant legislative frameworks on behalf of Bristol.
* Practice in accordance with Social Work England standards, the Professional Capabilities Framework (PCF) and Knowledge and Skills Statement (KSS) for adult social workers.
* Manage a complex caseload appropriate to your level of experience, ensuring effective prioritization within eligibility criteria and available resources.
* Undertake strengths-based assessments to identify outcomes and determine personal budgets.
* Develop, implement and review support plans in partnership with individuals and carers.
* Conduct risk assessments where safety or liberty is at risk, applying safeguarding procedures and legal literacy in:
1. Mental Capacity Act 2005
2. Care Act 2014
3. Mental Health Act 2007
* Promote positive risk-taking and least restrictive practice.
* Enable creative use of universal services, community resources, and technology-enabled care.
* Contribute to supervision and development of social care staff and undertake student supervision where required.
* Promote best practice in safeguarding adults and legislative application.
Information Sharing:
Ensure information sharing is compliant with:
1. Data Protection Act 1998
2. General Data Protection Regulation
3. Local safeguarding information-sharing protocols.
* Apply advanced knowledge of legislation, policy, and evidence-informed practice in all decision-making.
* Participation in rotas and ongoing CPD will be required to maintain specialist practice standards.
