Job overview

We are looking for an enthusiastic nurse to work within the Forensic Directorate. The post holder will be joining for our new physical health team as a Physical health nurse band 7. This is an exciting opportunity to work closely with staff and service users to improve the physical health outcomes for our service users within the forensic service. We are looking for someone who is keen to improve physical health in mental health nursing skills who will be working alongside a group of physical health professionals including dietitian, sports therapists, physical health nurses, pharmacists and working closely with inpatient teams. They will be responsible for running physical health clinics & working close with primary care services and supporting education and training needs for inpatient staff. The post holder will need to be able to work closely with the inpatient services to work and manage a caseload of service users & support primary nurses with physical health care planning.

Main duties of the job

The post holder will join a multidisciplinary NHS mental health service dedicated to providing secure services.

There are opportunities to participate in physical health audits and the educational program within the directorate. You will also be required to attend regular clinical supervision with a senior clinician.

The Physical Health nurse will carry out regular reviews on the in patients, supervised by a consultant psychiatrist, and liaise with the multidisciplinary team in the forensic services to deliver effective healthcare for patients. To also attend and contribute to weekly ward rounds, team formulation meetings and physical leads meetings.

The post holder will undertake a key role in the organisation and delivery of physical health care in forensic services an area which is increasingly recognised as essential in delivery of safe and effective mental health services. The post will involve liaising with the GP, Opticians, Dentists and all other local clinics for the service and delivery of effective healthcare for the management of acute and chronic physical conditions.

The Post holder will compliment roles already developed within the service and strengthen the multi professional team. The post holder will also work with occupational therapists, Tissue viability nurses, dietetics and nursing colleagues and liaise as needed with local Acute Hospitals.

Education/ Qualification/ Training

Essential criteria

1. Registered first level health care professional e.g. NMC registered Nurse (RGN), OT Diploma in LTC
2. First or higher degree in nursing related subject supplemented by post-graduate diploma specialist training and experience
3. Extended/Supplementary nurse prescriber
4. IAPT-LTC training

Experience

Essential criteria

5. •Experience of working within a multi-disciplinary team
6. •Experience working in a team leader role
7. •Experience of effective organisation planning and monitoring of service delivery
8. •Demonstrable experience and understanding of health and social care

Desirable criteria

9. •Post nursing registration experience
10. •Running Clozapine Clinic
11. •Experience of providing care in line with Long term condition agenda

12. •Ability to think strategically
13. •Evidence of knowledge in current professional issues
14. •Experience of presenting information to a wider audience
