Hospice Community Paramedic Practitioner (development post)

StElizabeth Hospice continues to drive forward innovation and development ofservices, both in the Ipswich and East Suffolk locality and further up thecoast to the Great Yarmouth and Waveney area.In 2019 NHS Norfolk and Waveney CCG awarded the specialist palliativeand end of life care contract to East Coast Community Healthcare. Thecontract includes a specialist palliative care service which will be deliveredin partnership with St Elizabeth Hospice.Our most recent Care Quality Commission report rated the hospiceas Outstanding.

InMarch 2023, St Elizabeth Hospice merged with East Coast Hospice to form StElizabeth East Coast Hospice, with all service provision in Great Yarmouth andWaveney coming under the St Elizabeth East Coast Hospice name.

Thisis an exciting time for St Elizabeth East Coast Hospice, as we are developingan achievable and sustainable palliative and end of life care service for theresidents of Great Yarmouth and Waveney. Its a time of expansion, developmentand investment in the Hospice Team. We areoffering an opportunity to be part of the Community Specialist Palliative CareTeam and also work within our Community Care Unit influencing the holistic careof patients and carers with end of life and specialist palliative care needs. This is a chance to make a real difference topeoples lives.


Main duties of the job

Are you an enthusiastic Paramedic Practitioner withdemonstrable experience and knowledge of providing care to people with aspecialist palliative care need?

We are looking for an experienced ParamedicPractitioner to join our Community Team.You will work withour multi-disciplinary team providing specialist palliative care to patientsand their families within a service that works every day and at all hours, byphone, by visits and advising and teaching others. The role will provide bothproactive and reactive clinical support for adults living with a progressiveillness.

Salary: WTE: £38,682.00 to £46,580.00band 6 or WTE:£47,810.00 to £54.710.00 band 7

Location:Great Yarmouth & Waveney area. Exact base to be confirmed at interview

Status: Permanentor fixed term

To work within a teamproviding specialist palliative care to patients, within a community setting,the hospice, and community units and care homes.

Supporting carers andother providers delivering care, through the advice line, training andeducation.

To undertake in-depthholistic assessments of patients with palliative care needs, regardless oftheir diagnosis, and plan care to meet patient outcomes.

To provide information,advice, psychological and spiritual care to patients, their families andcarers, liaising and referring on as necessary to appropriate healthcareprofessionals in the hospice, hospital and community

To support withadmission prevention where possible but to support timely admission forpatients with reversible causes.

To visit patients andfamilies when a specialist consultation is needed or seeing patients in theCommunity Unit when able to travel.

To provide telephonicsupport to patients, families and other professionals provided day and night.

To attend andparticipate in multi-disciplinary palliative care meetings around patient caremanagement, and present patients at the community MDT and to the Caseloadreviewer.

Provide a rapidresponse service for patients needing urgent specialist care. Providing care asneeded such as drugs, position changes and interventions, if appropriate toimprove the comfort of the patient, or advising others to do so.

To represent thehospice and patients at meetings, as required.

To maintain and promoteeffective and efficient working and communication with all involved in thepatient, to ensure appropriate sharing of information.

To provide support and expertise to other professionals,as required, acting as a role model and a resource providing expert knowledgeand support.

Tosupervise junior roles to be responsible for supervision, annual appraisalsand delegation of workload.

To comply with relevant drug policies, includingNon-Medical Prescribing and legislation concerning drugs and administration inthe future.

To attend Gold Standard Framework (GSF) Meetings.

To have an understanding of safeguarding principlesand be able to apply these within everyday practice and intervention/withinyour role.

To work with the CommunityTeam Leader, to plan and implement change and development within the CommunityTeam and wider Hospice team where this would increase effectiveness andefficiency of practice.

Tomanage patient caseloads, ensuring good communication, planning and reviews.

To keep up to date withbest practise, disseminate and embed the learning into patient care.

Lead the development of clinical standards,research and clinical audit, working within the Clinical Governance Frameworkof the Hospice and in palliative care in general.

To record all equipmentloaned out, ensuring records are kept up to date and that equipment isreturned.

To work autonomouslymaking decisions but seeking advice from colleagues or/and senior clinicalteam, as needed.

To deputise for theCommunity Team Leader in times of absence.

PERSONNELRESPONSIBILITIES

To participate inprofessional staff placements and lay persons to the community service.

Plan and deliverspecialist education and training e.g.; symptom control for GPs and DNs

To recogniseindications of staff stress and to facilitate staff support.

To work with the Community Team Leader, identifyingown educational needs.

Contribute to theidentification of training needs both internal and external.

To update ownspecialist skills and knowledge in cancer and palliative care, throughcontinuous professional development.

To demonstrate aresearch-based approach to care and work in conjunction with themulti-professional team towards applying this to practice.

To participate ininternal and external education programmes as required.

To give feedback fromattended education programmes/study days to the multi-professional Team.

To participate insupervision sessions/peer support, as required.

PERSONALDEVELOPMENT

To devise a personaldevelopment plan with the Team leader in line with the Hospice objectives andpersonal needs.

To maintain andincrease personal, clinical and managerial skills.

ORGANISATIONAL

To work at all timeswithin the policies and guidelines of the Hospice, participate in the ongoingreview and update of the policies and guidelines within the community team and actwithin HCPC guidelines at all times.

To ensure effective useof resources.

To play an active partin the development and maintenance of good relationships with all who havebusiness with the Hospice.

To respect at all timesthe confidentiality of information covering patients, staff and volunteers.

To promote the Hospicephilosophy of care towards patients, relatives and visitors.

Person Specification


Specific Skills

* High level of organisational skills.
* Able to prioritise and time manage work.
* Able to work independently and within a team.
* Able to analyse complex patient information.
* IT literate.


Qualifications

* Qualified Paramedic registered with the HCPC
* Educated to degree level
* Educated to Masters level or working towards, in palliative care or a similar related qualification.
* Educationalist qualification or proven teaching experience
* Advanced communication course
* Non-Medical Prescriber

* Confident in finding resolutions which may be challenging to implement.

Experience

* Minimum 2 years experience as a paramedic.
* Experienced in providing palliative advice and support to end of life patients, families and other professionals.
* Experienced in assessing patient needs that may be complex and subject to change.
* Good knowledge of community care.
* Working in the community and in the patients own home.
* Knowledge of the wider palliative care agenda.
* Advanced communication skills.
* Service Development.
