<div><h3>Clinical Fellow - Hospital@Home Frailty (ST1-ST3 Eq</h3><p>We are delighted to invite applications for a number of <b>Clinical Fellow</b> posts within our expanding <b>Frailty Hospital @ Home (H@H) </b>Virtual Ward service. This is an exciting opportunity to join a forward thinking, community based team delivering high quality, patient centred acute care outside the traditional hospital setting.</p><p><b>Contract Type</b>: Fixed Term: 5 August 2026 -- 3 August 2027</p><p><b>Hours</b>: Full Time - 40 hours per week</p><p><b>Working Pattern:</b> 08:30--16:30, Monday--Friday + weekend rota covering the working hours 0800-2000</p><p><b>On Call:</b> There is no on call requirement for this position</p><p><b>Base:</b> Community / Trust-wide</p><h3>Main duties of the job</h3><p>As a Clinical Fellow, you will play a key role in the clinical management of frail and medically unwell patients within their own homes. You will gain valuable exposure to frailty, complex comorbidity, and heart failure management -- all highly relevant to future training applications.</p><h3>Person Specification</h3><h3>Qualifications</h3><ul><li>Medical Degree: BSc, BA with grade 1 or 2:1, or equivalent</li><li>Full GMC with a Licence to Practice</li><li>Medical Degree: BSc, BA with grade 1 or 2:1, or equivalent</li><li>Full registration and license to practice with GMC</li></ul><h3>Experience</h3><ul><li>Completion of foundation training or equivalent, including evidence all clinical skills.</li><li>Experience of working in an NHS hospital, other than that of Clinical Attachments</li><li>Teaching of junior medical staff, nursing staff</li><li>Demonstrate a commitment to the specialty</li></ul><h3>Additional Criteria</h3><ul><li>Evidence of participation in audit / quality improvement locally and understanding of the principles</li><li>Appropriate skills for level of experience</li><li>Ability to work well in a team and under pressure</li><li>Good time management</li><li>Experience in the assessment and management of unselected medical emergencies at foundation level</li><li>Experience of acute medical speciality at foundation level</li><li>Evidence of a good standard of Literacy / English language skills</li><li>Ability to communicate well with patients, their relatives and colleagues</li><li>Willing and able to work with colleagues in management, nursing and other specialities to ensure the delivery of a high-quality, safe service</li><li>Familiar with clinical governance</li><li>Published work from BSc project</li><li>Other publications</li><li>Interpersonal sensitivity</li><li>Resilience</li><li>Experience in clinical governance</li></ul><h3>Disclosure and Barring Service Check</h3><p>This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.</p></div>
