Job Overview
An exciting opportunity has arisen for an enthusiastic and motivated Band 5 Registered Nurse to join the multi-disciplinary Home First Team based in Wrexham. The post is 37.5 hours, 5 days a week (10am-6pm). Part time or job share is also considered.
Responsibilities
* Assist in the emergency quadrant at Wrexham Maelor Hospital.
* Screen patients to facilitate safe and timely discharge from Emergency or Assessment Units via the Discharge to Assess model of care.
* Provide therapeutic interventions in the patient’s environment to promote the Home First ethos.
* Act as a link between acute and community services, working with Community Resource Teams in Wrexham and Flintshire.
* Communicate effectively with a multi-disciplinary team.
Qualifications & Requirements
Essential:
* NMC registered.
* Pre‑registration evidence in direct nursing care.
* Experience providing holistic nursing care.
* Clear understanding of contract of care, consent process and clinical governance framework.
* Ability to document details accurately and basic IT skills.
* Capacity to work under pressure.
Desirable:
* Post‑registration training evidence (e.g. POVA).
* Child protection experience.
* Intermediate life support.
* Knowledge of incident reporting policy and awareness of policies.
* Experience implementing evidence‑based practice.
* Welsh language ability.
