Main duties of the job
This post is based at North Walsham Memorial Hospital. You will be a member of the therapy team on the ward, working with registered Physiotherapists, Occupational Therapists, Assistant Practitioners and Therapy Healthcare Assistants.
* Deliver high‑quality physiotherapeutic interventions and health care to patients. Provide proactive, holistic case management and rehabilitation, helping patients set realistic goals. Use health‑coaching skills to engage patients and their families and manage expectations.
* Assess and prescribe care programmes for patients with long‑term conditions, the frail and elderly, palliative care and rehabilitation needs, aiming for quality of life and independence.
* Assess and prescribe exercise programmes for frail and elderly patients, palliative care and rehabilitation needs, to achieve quality and independence.
* Work within the integrated team to facilitate early discharge from hospital.
* Collaborate with health care professionals, statutory and non‑statutory agencies to provide a seamless integrated service.
* Provide advice, support and supervision to junior staff, therapy assistants and students.
Job responsibilities
Professional / Clinical responsibilities
* Accountable for own professional practice under the HCPC code of conduct; pursue ongoing development and attend mandatory training.
* Conduct comprehensive, holistic physiotherapy assessments, including for complex presentations, using investigative and clinical reasoning skills to form differential diagnoses.
* Plan and assess care programmes with realistic physiotherapy goals in consultation with patients, families and multidisciplinary teams.
* Obtain informed consent prior to interventions and respect patient privacy, dignity and courtesy.
* Provide information and education to enable patients to make healthier lifestyle choices; use alternative methods for those with hearing, mental capacity or comprehension difficulties.
* Promote effective communication within the multidisciplinary team, GPs, social services, voluntary services and other agencies; collaborate with them.
* Recommend innovative changes in clinical practice to enhance patient care.
* Understand Assistive Technology and make referrals as required.
* Participate in multi‑disciplinary and multi‑agency meetings, such as Gold Standard Framework meetings, and contribute to team meetings.
* Engage in audit processes and act on outcomes.
* Ensure workload for self and others is planned and prioritised according to service needs and targets; participate in central triage roster and patient allocation.
* Inform Line Manager / Clinical Lead of circumstances that may affect safe and competent practice.
Management and Leadership
* Provide clinical supervision to other team members and attend supervision meetings as per Trust policy.
* Assist with the induction of new staff and students; act as Clinical Educator where appropriate.
* Serve as a role model, providing guidance and support to other staff and students.
* Support all team members, respecting their roles and contributions.
* Be responsible for the safe use of equipment and supplies.
* Arrange travel to patients' homes, bases, meetings and training as required.
* Ensure efficient use of all team/ward resources.
* Lead all team members, promoting effective teamwork.
* Work within the budget and act to keep spend controlled.
* Assist in requisitioning supplies and equipment via electronic ordering systems.
* Account for work delegated to others.
* Participate in problem‑solving related to work organisation.

* Develop personal plans for junior members of staff.
* Ensure absence reporting is undertaken as required by line manager and Trust policy.
Information Management
* Keep clinical records confidential in accordance with Trust policy and follow computer security practices.
* Complete Information Governance training annually.
* Share information on a need‑to‑know basis and report breaches to the Line Manager.
* Document all patient care accurately, legibly and timely in information systems.
Education and Professional Development
* Keep Mandatory Training current.
* Maintain continuous professional development.
* Act responsibly according to professional body guidelines.
* Mentor students in pre‑ or post‑registration courses.
* Mentor existing staff and provide specialist training as appropriate.
* Provide training to students, new staff and less experienced colleagues.
* Participate in personal development review processes.
Risk and Governance Management
* Undergo and act on risk assessments.
* Report all incidents and risks per Trust policies.
Qualifications
* Diploma or first‑level degree in Physiotherapy.
* Current HCPC registration.
* Clinical Educator course or equivalent.
Experience
* Experience as a band 5 physiotherapist across a broad range of specialities (orthopaedics, MSK, neurology, care of the elderly) in acute and community settings.
* Experience performing falls assessments and gait analysis.
* Experience working with multidisciplinary teams and partner agencies.
* Experience managing patients with multiple pathologies and holistic, patient‑centred care.
Skills, Abilities and Knowledge
* Organise and prioritise workload independently and support others.
* Work on own initiative.
* Have attended short courses or in‑house training related to post‑registration physiotherapy.
* Good computer skills and familiarity with information management systems.
* Knowledge of safeguarding issues for vulnerable adults and older people.
* Relevant safeguarding training.
* Working knowledge of SystmOne.
* Experience of audit/research.
Communication
* Excellent verbal, written and interpersonal skills that support other team members.
Personal and People Development
* Evidence of personal development and experience.
* Support the development of others through supervision and teaching.
* Support patients in self‑management through health coaching.
* Clinical Educator qualification will be essential but can be gained while in post.
* Health Coaching training.

Working hours and conditions
* Post holder will work standard hours 8:30–16:30 Monday to Friday, with flexibility for extenuating circumstances.

* Must communicate effectively in written and verbal English.
* Hold a full, valid driving licence and have access to a vehicle for work.
