Overview
The Coordinator will promote and support the implementation of best practices in stroke care as identified by the Ontario Stroke Network. Works in collaboration with the QH Medicine & Critical Care Program, the Regional Stroke Centre, Regional Stroke Steering Committee and Quinte District Stroke Council. Provides leadership across the continuum to support integrated services to patients and families requiring stroke care. Identifies, facilitates and/or coordinates education for Health Care Providers in best practices in stroke care.
Responsibilities
* Provide leadership to achieve integrated stroke care throughout the geographical catchment area and across the continuum of stroke care, including health promotion, prevention, acute care, rehabilitation and community and long-term care.
* In collaboration with the Quinte District Stroke Council, support the implementation and evaluation of an integrated stroke delivery system, adopting best practices based on a model of continuous learning and continuous improvements in stroke care throughout the geographical catchment area.
* Work collaboratively with Quinte District Stroke Council in developing and supporting a comprehensive work plan for implementation of Best Practices in Stroke Care across the continuum and monitor progress. Update and maintain a current District Work Plan which feeds into the Regional Work Plan, outlining all best practice stroke care initiatives across the care continuum, including timeframes, current status, quality indicators, link to best practice standards and accountability.
* Work collaboratively with Regional and District Partners and other stakeholders to lead the development of an integrated stroke strategy, building upon existing services, networks and resources.
* Participate in the development and implementation of the stroke infrastructure within the four sites of QHC as a District Stroke Centre to support best practice, inpatient IPC team development and education, patient care and transition planning. Assist QHC four sites in localizing and implementing standardized stroke protocols, including developing and sustaining best practice service delivery within an Acute Stroke Unit.
* Project development, maintenance, performance monitoring, and leadership in evidence-based practices.
* Work collaboratively to provide strategic planning leadership, operational planning and budgeting, human resources selection, management and accountability; professional practice development; quality and risk management.
* Identify education needs and facilitate and coordinate education for health care providers in best practice stroke care. Develop and rollout of stroke best-practice physician and staff workshops.
* Actively participate on committees and working groups within the Medicine & Critical Care Program at QHC and with the Integrated Stroke Network at the District, Regional and Provincial levels.
* Foster and develop linkages and improve coordination of existing services across the continuum of stroke care. Coordinate and assist the community-based agencies responsible for health promotion, stroke prevention and reintegration in building inter-organizational relationships throughout their respective catchment areas and across the spectrum of stroke care.
* Work collaboratively with the Regional Stroke Centre and emergency services to develop and implement the acute stroke protocol as it relates specifically to ambulance dispatch communication policies, hospital bypass protocols, and community hospital triage and transport process.
* Provide leadership to achieve best practice preventative stroke care for high risk stroke patients within the Secondary Stroke Prevention Clinic, ensuring close linkages with primary care, acute care, rehabilitation, and community programs. Also responsible for ongoing SPC data collection and evaluation of performance indicators.
* In collaboration with the Regional Stroke Centre and other District Stroke Centres, establish evaluation frameworks to monitor utilization, financial and performance outcome indicators in order to demonstrate the impact of the Ontario Stroke Network throughout the geographical catchment area of the District Stroke Centre.
* Maximize opportunities for consistency and standardization of protocols and standards developed by the Regional Stroke Centres and the Ontario Stroke Network.
* Identify, facilitate, coordinate, and provide opportunities for public awareness and education.
* In collaboration with the Regional Stroke Centres, identify and pursue opportunities for pilot projects, proposal development and/or funding.
* Other duties as assigned.
Qualifications
* Member of a regulated health profession under the Regulated Health Professions Act.
* Active member status in the associated college.
* Familiarity with strategic planning, operational planning and budgeting and human resources management.
* Excellent project management skills with demonstrated ability to manage multiple projects and priorities.
* Demonstrated ability to work collaboratively with multiple groups and stakeholders.
* Proven flexibility with a high level of initiative and self-direction, and demonstrated ability to work independently in maintaining time commitments and achieving project outcomes.
* Demonstrated experience in evidence-driven performance improvement, including collection, interpretation and evaluation of data.
* Demonstrated familiarity with Ministry databases, including CIHI NACRS, NRS.
* Superior oral and written communication skills.
* Proven ability to lead and implement change.
* Previous clinical experience working with stroke patients.
* Proficient in computer applications and software, including Microsoft Office Suite (Word, PowerPoint, Excel, Outlook).
* Previous experience in providing educational opportunities to healthcare providers.
Preferred
* Master's Degree in a health-related discipline.
