Are you looking to work in an intervention-led service to promote wellbeing among older adults with mental health diagnoses? We are seeking Mental Health Nurses to join our older adult pathways of care, which deliver memory assessments and intervention-based care for individuals with serious mental illness and/or dementia with behavioral or psychological needs.
West Kent MHT+ is a diverse, multicultural team recruiting a Band 6 Mental Health Nurse with experience working with older adults. The successful candidate will provide specialist mental health care for people with complex needs, offering interventions that promote wellbeing and recovery within an adult all-age service.

Key Responsibilities:
* Act as a Practice Assessor
* Adhere to NMC registration and revalidation requirements
* Follow KMPT policies, procedures, and guidelines
* Uphold KMPT Trust values through professional behavior
* Assess, report, and act on safeguarding concerns
* Promote health and wellbeing, supporting self-care and involving carers when appropriate
* Manage and monitor medications as required
* Carry out duties related to community care depot and clozapine clinics
* Provide interventions to promote recovery
* Assess risks and identify client needs
* Monitor complex physical health needs
* Conduct cognitive and memory assessments
* Collaborate with other agencies involved in patient care
* Accurately record changes in behavior and risks
* Document care activities in approved KMPT systems
* Supervise and support team members
* Model professional behavior and mentor new staff
* Undertake ongoing training to maintain competency
