Role

To provide high‑quality support to people, mentor, coach and act as a strong role model for other staff in delivering high quality support in order to fulfil our mission.

Positive Futures for people with a learning disability, acquired brain injury or autism working together to achieve dreams and transform lives.


Responsibilities

* Form a positive relationship with each person supported, maintaining professional boundaries with the individual and their families.
* Provide high‑quality, person‑centred support at all times, drawing on the individual's portfolio and, where relevant, their Behaviour Support Plan.
* Support people to be safe and healthy:
o Assist with personal care, ensuring dignity and respect.
o Help people recognise and protect themselves from abuse.
* Promote independence, social and communication skills:
o Encourage choice and control over all aspects of their lives.
o Support informed decision‑making and advocate for self‑representation where needed.
o Clarify responsibilities related to decision‑making and lifestyle choices.
* Help people develop and maintain key skills:
o Manage home responsibilities in line with tenancy agreements.
o Assist with domestic chores such as shopping, meal preparation, cleaning, gardening, laundry and ironing.
o Encourage independent medication management and financial independence.
* Coaching, mentoring and supervision of staff:
o Co‑operate in induction, coaching and mentoring of staff in active support.
o Provide guidance on managing challenging behaviours using stress, coping and PBS plans.
o Support staff in applying person‑centred thinking tools to maximise choice and control.
o Act as shift leader, offering advice and guidance to colleagues when needed.


Qualifications

* At least 6 months paid experience supporting people in a health and social care or community setting.
* Experience leading a shift or acting as a key worker, supervising colleagues and coordinating reviews or plans.
* Confidence in using digital systems for records, rota, medication logs and incident reporting.
* Experience working in line with a Positive Behaviour Support plan (e.g., stress and coping plans or PBS plans).
* Effective oversight of paperwork and processes within the working environment.
* QCF/NVQ Level 3 in Health and Social Care or equivalent. Candidates without this qualification may apply if they are willing to undertake the qualification within an agreed timescale.

Salary

£25,500.00 – £27,500.00 per year
