Overview
We are looking for an ambitious Specialist Community Public Health Nurse (Health Visitor) to join our award-winning service in Somerset. Using your lived experience, you will be able to get involved in service development and share ideas and best practice. You must be an NMC registered Nurse or Midwife with a SCPHN qualification and an interest in holistically addressing the health needs of children aged 0-5 years and their families. We are working to improve the lives of children under 0-5 and their families in Somerset and you will be a key part of that.
* The salary for this post is in line with Agenda for Change Band 6, £30,945 to £37,264 per annum for 30 hours per week.
* Location: Burnham, Cheddar and Highbridge area (30 hours per week).
* Membership of the NHS Pension Scheme, subject to criteria, or Auto enrolment onto our Pension Scheme with optional pension enhancement through the Additional Voluntary Contribution scheme.

Main duties of the job
Your day-to-day work will involve:
1. Lead on the delivery of the Healthy Child Programme as defined within the service specification, identifying packages of care based on the Universal, Targeted and Specialist models of service delivery.
2. Provide leadership and supervision to other members of the Public Health Team, ensuring appropriate delegation and working within Somerset Local Authority policies and procedures.
3. Take responsibility for the outcomes of the children and collaborate with key partners to address identified public health needs.
4. Develop and foster effective professional relationships and networks with families and other key partners.
5. Provide families and children 0-5 with a service that promotes their physical, mental, emotional health and wellbeing.
6. Contribute to multi-agency early support assessment of children and families and take on lead professional role where appropriate.
7. Identify those children who are vulnerable and/or at risk from harm; respond to incidents of abuse and ensure child protection practice aligns with local and national guidelines and policies.
8. Attend professionally related working groups; participate in projects, pilots, research and audit as required by the Organisation.
9. Maintain knowledge of public health targets, delivery plans, governance plans and targets, ensuring practice reflects objectives.
10. Responsible for care plans/programmes delegated to the health visiting team; provide clear measurable outcomes and review systems for care.
11. Work in partnership with families and other agencies to promote healthy lifestyles and positive parenting; develop, deliver and evaluate education and support for targeted individuals and groups.
12. Provide safe and competent advice for individual clients; prescribe where appropriate from relevant formularies and qualifications.
13. Investigate and report complaints and incidents at team level and complete risk assessment in line with policy.
14. Offer advice, guidance and support to children and families to develop good practice and publicise available assistance.

Communicate in person, in writing and, where appropriate, electronically with all contacts.
* Establish clear lines of communication within multi-agency and professional Primary Care teams.
* Liaise with other agencies to keep them informed of service provision and with consent, advise on individual cases.
* Identify improvements to service provision, policies and procedures.
* Coordinate inter-agency/partnership working to enhance access to services and provide comprehensive service provision.
* Attend Area team meetings monthly to support effective communication within children and young people’s services.
Resources
* Manage the local team within agreed resources (financial and personnel), including physical resources, to enable early intervention.
* Monitor and review loans of equipment in line with protocols and guidelines.
* Prioritise work and manage time effectively; coordinate monthly team meetings.
* Support skill development through supervision, appraisal and personal development planning; act as an assessor for students and provide public health knowledge; deliver training as appropriate.
* Recommend and implement improvements in service delivery and practice.
* Provide and participate in clinical supervision as per policy and guidelines.

Qualifications and Experience
Experience (Essential):
* Experience of working across agencies; understanding and application of research in practice; evidence of child protection experience; experience in identifying and assessing health needs including community health needs.
Qualifications (Essential):
* Registered Health Visitor Public Health Specialist Practitioner Degree (or equivalent pre-1988) and evidence of continued professional development.
Desirable:
* Leadership programme or willingness to undertake a course; Nurse Prescriber V100 qualification or equivalent.
* Audit and research skills.

Knowledge and Skills (Essential):
* Public Health and Preventative Strategy knowledge; knowledge of child protection procedures and policies; ability to organise and deliver education in 1-1 or group settings; ability to manage a caseload and delegate; willingness to lead teams; clinical and managerial leadership; report writing; IT competence; maintain records per NMC guidelines.
* Desirable: Audit and research skills.
