Responsibilities
* Work with adults in need over the age of 18, actively listening, collaborating and tailoring plans to enhance well‑being.
* Partner with healthcare providers to prevent crises, emphasising existing support for independence.
* Seek local, sustainable solutions and offer diverse intervention and prevention opportunities.
Qualifications
* Registration with Social Work England.
* Strong interpersonal skills and effective communication.
* Updated knowledge of relevant legislation, safeguarding vulnerable adults, and Health and Social Care legislation and policy context.

* Hybrid working model: office staff on site at least half their hours; community‑based staff conduct at least half admin time in a workplace; customer‑facing roles work entirely from service location.
