Ackerman Pierce is currently supporting a leading local authority within Sutton to recruit for a Social Worker within the Leaving Care & Children Looked After team within the Social Care Industry on a permanent basis paying £42,060-£50,085 P/Annum. Main duties include: Undertake timely assessments and analysis of need and risk, working with families and networks of professionals to create and deliver outcome focussed plans. Ensure that children and young people and carers are aware of their rights and are enabled to participate meaningfully in planning, and in decision making including through the use of independent advocates if required. Provide support and intervention based on best evidence to meet individual child and family needs. Secure access to services, negotiating and challenging other professionals and organisations to provide the help required.
