The role:

The purpose of this role is to support the HSCP Prescribing Team in promoting safe, effective and economic use of medicines through working with pharmacists, GP practices, Care Homes and Health and Social Care Staff. As a key member of the Prescribing Team, you will lead and support pharmacy staff, promote evidence-based prescribing, and work collaboratively with multidisciplinary teams across GP practices, community pharmacies, care homes, and social care settings.

Duration, Location, and Working Pattern:

1. An exciting chance for an enthusiastic and self-motivated pharmacy technician to join the primary care pharmacy team in the North-West Locality Glasgow City to support work in primary care in GP practices & Care Homes and delivering safe, effective, and person-centred care
2. This role covers the Northwest of the city and will predominantly be based at our Pharmacy Hub site in Borron Street
3. The shift pattern for this post is Monday to Friday

Key responsibilities:

4. To collect and collate patient-related prescribing and clinical data from practice computers and/or patients notes
5. To identify and highlight areas where patient prescribing management requires intervention and to make relevant recommendations in order to support reviews related to HSCP priority areas
6. To create reports/letters for GP practices detailing work undertaken, outcomes and recommendations e.g. audits, patient data collection and staff training
7. To implement agreed prescribing change to ensure high quality and cost effective prescribing
8. To input prescribing data on appropriate databases e.g. prescribing queries, information required for HSCPs reports
9. In conjunction with the HSCP pharmacists implement an action plan with the GP practice team and provide any relevant associated support that the GP practice may require
10. To support practices in performing audits related to medicines use and patient reviews in line with HSCP and/or GP practice priorities
11. To facilitate the review of GP practice repeat prescribing systems at individual GP practice and HSCP level and to monitor standards with the GP practice team
12. To provide training to GP practice teams, clinical staff, Health and Social Care Staff and other multidisciplinary staff as appropriate on the safe and efficient management of medicines (e.g. repeat prescribing systems)
13. To support GP practice staff in using the practice computer system in relation to prescribing management issues by sharing good practice
14. To support the medicines reconciliation process in response to Immediate Discharge Letters, clinic letters, and other communication from secondary care or other specialist care providers in conjunction with the GP practice, Community Pharmacy and/or HSCP Pharmacist
15. To support review of an agreed list of targeted acute/ special requests, for authorisation by an independent prescriber

Knowledge, training, qualifications and/or experience required to do the job:

Qualifications

17. Pharmacy Technician, NVQ3 or BTEC in Pharmaceutical Sciences or equivalent
18. Registrant with the General Pharmaceutical Council
19. Minimum two years postregistration
20. Diploma in specialist pharmaceutical area

Experience

21. Portfolio demonstrating competence in previous and current pharmacy practice skills, knowledge and continuing professional development
22. Extensive postqualification experience in hospital, community pharmacy or primary care
23. Evidence of ability to work proactively with limited supervision
24. Audit and project work

Knowledge

25. Basic knowledge of the role of HSCP in provision of healthcare
26. Clinical understanding of medicines use
27. Awareness of law and good practice related to prescribing, storage and handling of pharmaceuticals
28. Knowledge of Pharmaceuticals and Therapeutics

Skills

29. Good analytical skills
30. Excellent written and verbal skills
31. Excellent interpersonal skills
32. Computer literate with good IT skills
33. Able to maintain strict confidentiality regarding patient and GP practice information

Ability

34. Prepare and present information in a userfriendly manner
35. Use own initiative to deal with a range of people
36. Take responsibility for pharmaceutical projects such as audits and surveys
37. Work independently and as part of a multidisciplinary team
38. Valid driving licence
