Offer description

As a Substance Care Practitioner in the Substance Care Team (SCT), you will be involved in alcohol and drug screening, brief interventions, fibro-scanning patients, and treating acute alcohol withdrawal following NICE-based protocols for managing alcohol misuse. You will also deliver Substance Care training to nursing, medical, and other frontline hospital staff.


Main duties of the job

As an integral member of the SCT, you will:

1. Help improve the identification and management of substance misuse across all wards and departments, aiming to reduce A&E attendances and inpatient admissions.
2. Provide assessment, specialist alcohol advice, and treatment to patients.
3. Support and train medical and nursing staff to enhance their skills in identifying and managing substance misuse.
4. Facilitate safe transitions of care between hospital and community settings, ensuring pharmacological treatments are transferred safely.
5. Promote opportunities for sustained recovery from alcohol misuse through effective care planning.
6. Liaise with health and social care professionals to ensure high-quality, coordinated patient care.

Job responsibilities


Key requirements

* Utilise SCT screening in triage and clinical assessments in emergency and ward settings.
* Follow referral procedures for timely clinic referrals.
* Deliver high-quality assessments and evidence-based interventions.
* Perform diagnostic fibro-scanning and liaise with medical teams.
* Implement NICE-based treatment programmes for alcohol misuse, including detoxification and recovery support.
* Ensure smooth care transitions for detoxification and aftercare, coordinating with community and primary care services.
* Enhance care pathways between hospital and community.
* Provide brief and extended advice in partnership with MDTs.
* Act as a resource for patients, visitors, and staff, and report feedback.
* Maintain patient records and participate in MDT discussions and quality projects.
* Signpost patients to external agencies for substance misuse support.
* Review and help embed assessment, treatment, and care policies.
* Support the clinical team with complex cases and sensitive information.
